This includes all items in the make-table that would be required to be date-marked - nothing is marked. COS - employee called the owner, and the owner told him that everything was removed from the freezer just yesterday. He dated the items accordingly. KDA_2_30114I - --Employee handles cash from customers, then dons single-use gloves and places egg rolls in a serving box with clean tongs - no hand wash in between. COS - educated KDA_3_20111B - --Employee states that the cookie bars, wrapped and on the front counter for customer self-service, are prepared in the owner's private home. COS - removed from retail sale KDA_3_30212 - --Two squeeze-bottles of oil, and one squeeze-bottle of soy sauce, are not labeled to identify contents. KDA_3_30412B - --A plastic cup is being used as a scoop in the dry rice bin - no handle to keep above the food. KDA_3_50111 - --"Frozen" foods stored in the glass-door ice freezer are soft and mushy to the touch - not stored frozen. KDA_3_50116A1 - --Chicken wings, hot holding in the display case, are 114.4F. COS by VD KDA_3_60211B4 - --Ice, bagged in the facility, has no business information included on the label. KDA_4_30212A - --Employee has no idea where a thermometer might be, or if they have one. KDA_4_50116A - --Employee states that they always wash their hands in the three-compartment warewashing sink. COS - educated that this is not allowed KDA_4_90311A - --Cardboard cases of single-service beverage cups and food containers are directly on the floor in a corner of the store behind a curtain - not 6 inches above floor as required. KDA_6_20214 - --There is no working self-closing device on the employee toilet room door. KDA_6_30111 - --There is no hand soap at the only hand sink in the food prep area. COS - employee provided hand soap KDA_6_50111 - --There are two badly-stained ceiling tiles in the dry storage area, with large buckets on the shelf directly below them. On a lower shelf, is a container of dry rice. KDA_7_10211 - --A spray bottle containing a purple cleaning solution of some sort is in the cabinet under the hand sink, with no label identifying contents. The employee on duty has no idea what it is. COS by pouring out KDA_7_20612 - --Partial bars, chunks, and crumbs of green rodent bait are on the floor in the cabinet below the customer hand sink, and on the shelf near the carbonater in the cabinet below the fountain machine. KDA_7_20911 - --Medicines and dietary supplements, for the owner's personal care per employee, are stored on the display shelves along with energy supplements for retail sale. KDA_Defacto - --All food for personal use must be segregated from food for food service operations in the make-table, and marked to indicate that it is "Personal" only. --Egg rolls, previously cooked, heated to 185F --Hot display case - meat pizza 145F --MT - sausage pizza topping 37.4F, chicken wings 38.3F WIC - milk 43.4F, chili dog 41.7F --The only employee on duty appears to have no knowledge of food safety - unable to answer any questions regarding dates, temperatures, homemade food being sold, proper handwashing, proper use of sinks, chemical identity, etc. Most of inspector's questions were answered by him calling the store owner and asking. Please review the code requirement, 2-102.11, "The PERSON IN CHARGE shall demonstrate knowledge of food safety appropriate for the operation by complying with the Food Code, having a current food protection manager certification, or correctly answering FoodSafety questions".	01/21/2016	19	Regular	Out
