3-303.11 - --Located in the bar area, the ice cooling coils were in direct contact with the ice. Drinks were being made and ice was being used for human consumption. COS. Educated staff on not using the ice for any type of eating or drinking. A large bowl was placed on top of the ice and fresh ice was placed inside the bowl to be used for drinks.
KDA_3_50116A2 - --At 4:30 p.m. The make table was not holding PHF foods at 41F or below. Diced tomato 54.6F
KDA_4_20211A2 - --Located on the shelving for clean dishes, there was one china bowl out of approximately 100 with chips on the rim. COS. Bowl was placed in trash can.
KDA_4_204112B - --The make table did not have a thermometer. COS. PIC found a thermometer and placed it in a jar of water and inside the make table.
KDA_4_50111A - --The ice machine lid is broken and cracked and has duct tape and is not cleanable. The counter area below the food service window is broken and chipped and is not cleanable.
KDA_4_50111B - --All of the RIC, RIF and WIC have broken and missing gaskets around the doors and the doors do not shut properly.
KDA_4_60111C - --The ice machine roof and the beer RIC have an accumulation of rust debris. The rubber holder for the soda nozzle has an accumulation of pink slimy mold residue. The container for flour and breading have an accumulation of dried food debris. The WIC door and handle have an accumulation of black sticky debris. The 4 white RIC and the chest RIF have an accumulation of black sticky debris on the doors.
KDA_6_50111 - --The ceiling light covers in the kitchen are cracked and chipped. The ceiling area in the kitchen and dining are warped and stained and need cleaned or painted.
KDA_6_50112A - --The door to the hot water tank room, the swinging doors between the kitchen and bar, all have an accumulation of black sticky debris. The ceiling fan in the kitchen has an accumulation of lent debris. The ceiling vents have an accumulation of lent and grease debris. All of the light switches in the FE have an accumulation of black sticky debris. The WIC floors, walls, and ceiling have an accumulation of dirt and sticky debris. The floors in the kitchen and in the bar area have an accumulation of black sticky debris.
KDA_6_50116 - --There is a wet mop laying in the mop sink all scrunched up and not hanging to air dry.
KDA_7_10211 - --Located in the bar area there were 2 spray bottles of blue liquid with no label. PIC said they were glass cleaner. COS. Bottles were labeled. Located in the kitchen there was one spray bottle of clear liquid with no label. PIC said it was bleach water. COS. Bottle was labeled. Located in the kitchen area, there was a wiping cloth bucket with liquid and no label. PIC said it was bleach water. COS. Bucket was labeled.
KDA_7_20111B - --Located in the bar area there was one spray bottle of glass cleaner sitting on top of the soda syrup. COS. Bottle was moved. Educated staff.
