- KDA_3_30511A1 - --In the WIF a box of donuts and other foods are stored directly on the floor.
- KDA_3_50116A2 - --Potentially hazardous foods in the make table in the kitchen were not held at 41 F or below. The following items had the following temperatures: ground beef 48 F ground sausage 49 F marinara sauce 47 F yellow shredded cheese 60 F sliced cheese 51 F salsa 55 F cut lettuce 61 F PIC said items had been in the make table since 10/06 or before. COS, VD. Also, a bag of white shredded cheese 55 F and a tub of white shredded cheese 47 F were in the make table. At 2:45 PM the PIC said she put the cheese in the make table from the WIC at approximately 1:30 PM. COS, moved back to WIC.
- KDA_4_20211A2 - --The lid to a food storage container has an area that is melted and the handle is broken making it difficult to clean. COS, VD.
- KDA_4_30111 - --The make table is not holding potentially hazardous foods at 41 F or below. Ambient 51.4 F
- KDA_4_50111B - --The seals on the MT doors are cracked.
- KDA_4_60111A - --The utensils are stored in holders in drawers. The utensil holders had food debris in them. The plastic handle of a spoon had a greasy residue on it. COS, moved to be cleaned and sanitized.
- KDA_4_60211E4 - --There is mold on the inside of the ice maker.
- KDA_4_90311A - --In the kitchen next to the make table a box of single use food storage containers were stored directly on the floor. COS, moved.
- KDA_5_20213 - --The hose attached to the mop sink faucet was hangling below the flood level rim of the sink basin. COS, removed.
- KDA_5_20511B - --There was a sheet pan covering the sink basin of the handwashing sink in the kitchen. COS, moved.
- KDA_6_30114 - --There is no sign to notify employees to wash their hands at the bathroom handwashing sink. Sticker left by inspector.
- KDA_6_50112A - --There is dust accumulation on the ceiling in the kitchen over food prep areas and the warewashing area.
- KDA_7_20111B - --In the kitchen the soap dispenser is mounted on the wall directly over a food storage shelf with bagged olives and other foods. COS, soap dispenser moved.
