Located on the sandwich MT, there were unlabeled shakers of white powder, black powder, and gray powder which an employee identified as salt, pepper, and a mix of salt and pepper. Located on a shelf under the pizza dough roller, there was an unlabeled container of white powder which an employee identified as flour. Located in the pizza MT, there was a container of shredded cheese with a cup with no handle in it for use as a scoop. KDA_3_60211B1 - --Located at the front counter, there were 2 sugar cookies, 5 frosted sugar cookies, 7 chocolate chip cookies, and 1 oatmeal cookie baked and packaged on site that were not labeled with their common names. COS- labeled all but the sugar and frosted sugar cookies. There were no labels available for these types, so cookies were pulled from front of counter. Corrected on site (COS)- moved to wash area. Located on a shelf on the back wall of the kitchen, there was a knife and an attachment for a vegetable dicer stored as clean that were soiled with dried food debris. COS- moved to wash area. Located on the clean dish storage rack in the kitchen, there were six plastic food container lids that had sticker residue on them. COS- moved to wash area. KDA_4_60111C - --The fan guards of the kitchen WIC has an accumulation of dust. KDA_4_60211E4 - --Located at the retail customer self serve area soda fountain, there is mold under the soda nozzles and on the ice chute. KDA_4_70211 - --Observed employee wash, rinse, and sanitize food containers in three vat sink. Tested concentration of QT sanitizer in three vat sink, was 100ppm. Label on QT sanitizer bottle indicated that proper concentration was to be between 200 and 400ppm. Corrected on site (COS)- employee remixed sanitizer to 300ppm and rewashed, rinsed, and sanitized food containers. KDA_4_90311A - --Located in the storage area next to the customer self serve area, there was a box of single use drink cups stored on the floor.
