- KDA_3_30414B1 - --There are wet rags being stored on the food prep tables during pauses in use. Advised to place in sanitizing bucket.
- KDA_3_50116A2 - --In the WIC,spaghetti was being held at 46.1F, marinara was being held at 44.9F,baked potatoes at 45.0F, cooked sausage at 47.4F, vegetable soup at 45.0F, cheddar cheese at 46.5F, tzatziki sauce at 45.0F, milk at 45.8F, and shredded lettuce at 46.3F. COS-dated Note-less than 10% of all PHFs in the establishment were date marked.
- KDA_4_20211A2 - --There were several plastic food containers stacked and stored as clean with cracks in them. Advised to remove.
- KDA_4_30111 - --The WIC is not holding PHFs at 41F or below. The ambient temperature measured 47.1F. Advised not to use the unit until it can be adjusted.
- KDA_4_60111A - --The meat slicer had dried food debris on the blade. The PIC stated it had not been used today. COS-recleaned.
- KDA_5_20515B - --The mop sink faucet leaks while not in use. The cap to the AVB on the mop sink is also missing.
- KDA_6_501111A - --There was one live roach (the a casing attached) under the steam table in the kitchen. Inspector spoke with the pest control company and they have been to the establishment 1 month prior.
- KDA_6_50112A - --The floor in the kitchen has old food and food splatters on the walls. Advised to clean. The floor drain to the 3 compartment sink has slime chunks in it. Advised to clean.
- KDA_7_20612 - --First Strike Soft Bait (EPA reg no. 7173-258) was on the floor. This bait is not in a tamper resistant container. Advised to remove.
