  • KDA_3_50116A2 - --In the produce display cooler bagged salads and shredded cabbage measured anywhere from 47.8F to 52.2F 3:15pm. The PIC stated they had been in the cooler since 9 am. COS-voluntary destruction (VD) and removed the cardboard from under the salads blocking the air flow
  • KDA_3_50118A3 - --In the deli end cap cooler, 7 layer dip (chopped tomatoes, beans,sour cream, salsa, and cheese) was dated 11/20/15 thru 11/27/15. It was dated for 8 days.

  • KDA_3_10111 - --There was a 20 pound bag of sugar that had been torn open and taped on the retail shelves. COS-VD A can of bamboo shoots had a black substance leaking out of a perforation in the can. COS-discarded
  • KDA_3_20215 - --Approximately 20 cans of beans, raviolis, and corn has class II dents in them. COS-PIC removed from shelves.
  • KDA_3_30412C - --The meat department knives are being stored inbetween the wall and 3 compartment sink. This area is not an easily cleanable surface. COS-PIC recleaned knives and placed them on a clean surface.
  • KDA_3_50113A - --Several whole turkeys were being thawed in the 3 compartment sink of the meat department. COS-meat dept manager placed them into the WIC.
  • KDA_3_50116A2 - --In the produce display cooler bagged salads and shredded cabbage measured anywhere from 46.7F to 50.3F (2pm). The PIC stated they had been in the cooler since 9 am. The ambient temperature of the cooler measured 46.1F. COS-voluntary destruction (VD)
  • KDA_3_50118A1 - --Oven roasted chicken opened on 11/2/15, garlic chicken opened on 10/24/15, cooked beef opened on 10/30/15 and turkey opened on 11/2/15 were in the deli display cooler. They had been held greater than 7 days. COS-VD In the deli make table, canned mushrooms were opened 10/28/15, ready to eat (RTE) sausage was opened 10/28/15, and RTE ground beef was opened 10/28/15. They had been held greater than 7 days. COS-VD
  • KDA_3_50118A2 - --In the deli make table, ham was without date marking. The PIC stated it was opened on 11/8/15. COS-dated
  • KDA_4_20211A2 - --Several spatulas in the deli were pitted and cracked. COS-discarded
  • KDA_4_30111 - --The produce display cooler is not holding PHFs at 41F or below. The ambient temperature measured 46.1F. COS- pulled PHFs from cooler and is not going to use it until it can be fixed
  • KDA_4_60111A - --The blade of the South slicer in the deli had dried food debris. The person in charge (PIC) stated it had not been used today. Advised to clean. COS-recleaned
  • KDA_5_20511B - --The deli handwashing sink was being used as a catch all sink. There were cans and various items stacked in the sink making it so one cannot wash their hands. Corrected on site (COS)- removed items from sink
  • KDA_5_20515B - --At the meat departments 3 compartment sink, the south faucet is leaking and is being taped to help control the leak. Advised to repair the leak.
  • KDA_5_40211 - --Several turkeys were being thawed in the meat departments 3 compartment sink that is directly plumbed. COS-moved turkeys to the WIC.
  • KDA_6_30111 - --The deli and meat department handwashing sink did not have paper towels. COS-supplied
