- KDA_3_10111 - --In the Bob Evans gravy machine, the gravy bag was burnt/melted and had a hole in the plastic bag exposing the gravy. Corrected on site (COS): The employee turned off the machine and will voluntarily discard the gravy when it cools.
- KDA_3_30212 - --A spray bottle setting on top of the make table with a clear liquid inside had no common name on the container. COS: Employee said it was water and labeled the container.
- KDA_3_30414B1 - --In the 3 compartment sink a bucket with clear liquid and wiping cloth had no detectable sanitizer. COS: Employee dumped the bucket and will remake it when needed.
- KDA_3_30511A1 - --3 packages of large pizza boxes directly on the floor. COS: Employee placed them on top of the chest freezer.
- KDA_3_50118A1 - --In the bottom of the make table, an open bag of mozzarella cheese had no date marking, but on the lid of the make table there was a date of 12/12. COS: The employee voluntarily discarded the bag in the trash can.
- KDA_4_20211A2 - --Under the pizza oven, a steak knife had a broken handle and was not smooth and easily cleanable. COS: Employee voluntarily discarded it in the trash can.
- KDA_4_30212B - --There is no probe thermometer in the establishment that can measure 41F. The only probe thermometer reads 50F-550F. Inspector suggests getting a thermometer that reads 41F to check cold holding temperatures and 165F to check for reheating temperatures.
- KDA_4_60111A - --On the counter by the hand sink, the dicer had dried on food debris. COS: Employee placed it in the 3 compartment sink to wash. The pizza peel under the pizza oven had dried on food debris. COS: Employee placed it in the 3 compartment sink to wash. Under the pizza oven, on the Hunt Brothers cart, a box of parchment paper had dried food debris inside the box and on the papers. COS: Employee voluntarily discarded them in the trash can. The F`real machine has a build up of food debris on the top inside. The cappuccino machine has food debris build up on the dispensing cups and food debris on the inside of the machine.
- KDA_4_60111C - --In the bottom of the chest freezer there is a build up of food debris. The make table door seal has food debris build up.
- KDA_4_90311C - --The single use pizza trays setting on the pizza oven were not inverted. COS: Employee inverted them.
- KDA_6_50112A - --The walk in cooler fan guards are getting dust built up on them.
- KDA_7_20111A - --In the backroom storage area, on the shelving a box of HEET gas line antifreeze was stored next to/touching single-use F`real lids and straws. COS: Employee moved the chemical.
- KDA_Defacto - --Establishment has a small 2 gallon water heater. Inspector suggests installing a larger tank to maintain 100F water at the hand sink, 2 restroom hand sinks, and the 3 compartment sink. --Gravy: 149.1F --Walk in Cooler Burrito: 38.9F Make Table Pepperoni: 42.7F Shredded Cheese: 39.6F Summit Refrigerator Eggs: 43.5F Ambient Air: 38.1F Reach in Cooler Doors Milk: 40.1F Eggs: 36.7F
