  • KDA_3_30211A1b - --In the RIC (below the make table) there were 2 plastic containers of raw ground beef being stored in between ready to eat squeeze bottles of dressing and a round plastic container of nacho cheese sauce. COS-rearranged items. (Note: no leakage and both containers with the raw ground beef had closed lids).
  • KDA_3_30212 - --On top of the Maytag RIF, there was a round plastic food grade container with a white powder substance and not labeled. The PIC said it was flour.
  • KDA_3_30511A1 - --In the dry food storage area there was one box of potatoes being stored directly on the floor.
  • KDA_3_40311D - --In a crock pot there was taco meat being reheated. The person in charge said he placed it in the crock pot around 9-9:30am with the temperature setting on warm. It tempted at 52.8F around 10:30am (less than 2 hours). COS-rapidly reheated the product to 165F on the stove top.
  • KDA_3_50116A2 - --In a cooler with ice, there was a plastic container of cottage cheese at 45.9F. It was placed in the cooler around 8:30am and was tempted around 11:10am (less than 4 hours). The container was not submerged down in the ice. COS-placed in another cold holding unit. In the WIC there was a plastic container of cheesy potatoes (made on site) at 52.5F. There were packages of orange american cheese at 51.0F and 50.9F and white american cheese at 51.1F. There were packages of shredded cabbage (for coleslaw) at 50.1F and 49.6F. There was a box of individual packaged spaghetti noodles at 48.9F. There was a case and individual packets of whipped spread (keep refrigerated) at 52.3F. There were cartons of butter milk at 49.8F, 50.0F, 49.1F, 49.1F, 51.6F, 51.2F, 50.9F, 50.1F, 60.0F, 50.3F, 50.1F, 49.6F, 49.8F, 49.1F and 49.4F. There were 3 tubs of french onion dip (keep refrigerated) at 51.9F, 51.3F and 51.4F. There were packages of mild yellow cheddar cheese at 51.3F, 50.8F, 50.6F, 50.1F, 49.8F, 51.2F and 51.8F. There were packages of cream cheese at 50.5F, 49.5F, 49.6F, 49.8F, 51.2F and 51.8F. There were 2 jars of horseradish (keep refrigerated) at 52.3F and 52.9F. There was a package of blue cheese at 52.7F. There was a box of butter (keep refrigerated) at 51.8F. There was a plastic container of salsa (keep refrigerated) at 51.6F. COS-discarded all of the PHF from the unit. The ambient air temperature of the unit was reading 49.9F. (Note: the PIC called for the unit to be serviced and the part has been ordered).
  • KDA_3_50118A1 - --In the walk in cooler there was a plastic food grade container of hot sauce (made on site) with a make date of 12/31/15 (held past 7 days). COS-discarded. In the make table there was a squeeze bottle of mexican ranch dressing (made on site) with a make date of 1/2/16. The PIC said this was not the correct date, correct date was 1/8/16. COS-put the correct date on the bottle. In the RIC (below the MT) there was a ziplock bag of sliced turkey with a frozen date of 12/15/15 and a thaw date of 1/2/16 (held past 7 days). COS-discarded.
  • KDA_3_50118A2 - --In the make table there were plastic food grade containers of diced tomatoes, sliced tomatoes, cut lettuce leaves and shredded lettuce without dates. The PIC said the items were prepped on 1/9/16. COS-discarded the lettuce items and date marked the tomatoes.
  • KDA_4_20211A2 - --In a plastic container on a lower shelf across from the stove, there were 4 rubber spatulas (2 blue, 2 clear) with cracked, rough edges. COS-set aside to trim smooth. In the utility room on top of the ice machine, there were 2 plastic ice scoops that were cracked by the handle. COS-discarded.
  • KDA_4_30111 - --The walk in cooler ambient air temperature is reading 49.9F. (Note: the PIC called for the unit to be serviced, the part is order and will be fixed on 1/13/16).
  • KDA_4_60111A - --In containers on a lower shelf across from the stove, there was a black plastic spatula with dried food debris on the food contact surface,a black plastic spoon with dried food debris, one metal tong with dried food debris and one wire strainer with dried food debris and all were stored clean. COS-moved to the ware wash area. On a shelf above the 3 compartment sink there were four large griddles with dried food and grease build up on the sides and stacked clean.
  • KDA_4_60111C - --There is grease build up on the hood filters above the stove/fryer.
  • KDA_4_60212B - --The Sharp microwave had dried food debris build up on the inside walls and ceiling.
  • KDA_4_90311A - --In the utility room there was a package of plastic to-go containers being stored directly on the floor. In the dry food storage room there was one box of lids and one box of foam hinged lid containers being stored directly on the floor.
  • KDA_4_90311B - --On the lower shelf across from the stove, there were several utensils facing upwards.
  • KDA_5_20511B - --There were metal spoons and forks being stored in the handsink in the kitchen. COS-removed the utensils.
  • KDA_5_20515B - --The handsink located in kitchen drips water when the water is shut off. The base of the faucet located at the 3 compartment sink in the kitchen leaks when water is supplied. The base of the faucet located at the 3 compartment sink behind the bar leaks when water is supplied.
  • KDA_6_20215A3 - --There is a 1/4" gap at the bottom left corner at the west side exit door. This could allow the entrance for pests.
  • KDA_6_50112A - --There is food beneath the grill/fryer. There is trash and boxes on the floor in the utility room.
  • KDA_6_50116 - --The mop head was resting in the utility sink (not able to air dry).
