Observation: Coving/baseboard is hanging off of the wall under the 3 compartment sink and is missing in other ares in the ware washing area.

Observation:Exteriors and door handles of the 4 door pizza prep refrigeration unit and the 2 door refrigerated salad make table were soiled with food residue. The top of the dishmachine is excessively soiled with residue.

Observation:Portion cups used as scoops were observed stored in a container of food product in the walkin cooler.
- PHF/TCS food maintained at 135°F or above, except during preparation, cooking, cooling, or when time us used as a public health control
Observation: BBQ chicken, mashed potatoes, and cooked green beans were hot held at 103*F on the buffet line. Product was removed by the manager and disposed of.CORRECTED AT TIME OF INSPECTION.

Observation:Build up of food residue below equipment and around equipment legs in the kitchen. The wall underneath and behind the 3 compartment sink is dirty.

Observation:Unlabeled container of white food substance that appeared to be Parmesan cheese (in walk in cooler).

Observation: Broken light shields in the kitchen and the light shield above the raw chicken prep area is not secured to prevent shattered glass from falling onto food below.

Observation:Employee that was clearing tables and disposing of remaining food in the trash, took a bite out of a piece of pizza whiled dumping food in the trash in the warewashing area. Employee threw the remaining piece of pizza in the trash.
- Food employees hair is effectively restrained
Observation:Food handler was not wearing a hair restraint. Food handler preparing chicken was not wearing an adequate hair restraint to enclose the hair on top of the head.
