Observation: Portion bowl being used as scoop in blue cheese container in kitchen line reach-in cooler. Must utensil with handle stored outside of food and keep inverted to prevent contamination.

Observation: Food employee was not wearing hair restrains. Food handling employee must wear hat or hair net when working around food. Corrected.

Observation: QA dispenser was not working. QA concentration must be maintained 200-400ppm. Fix.

Observation: Paper towel dispenser was empty during the time of inspection. Must provide paper towels during all business hours. Corrected.

Observation: Ice buildup on the floor in walk-in freezer in basement. Din't see any leaks at time of inspection. Clean. Equipment must be maintained in good working order.

Observation: Visible buildup on can opener. Clean more frequently to prevent contamination.
