- Light intensity
Observation: Have no light or missing light bulbs in following coolers: 1) Beverage air reach-in cooler, 2) Maximum cooler in the kitchen. Must provide adequate light intensity in the area where is food stored inside reach-in refrigerators for easily cleaning and maintaining.
- Established procedures for responding to vomiting and diarrheal events
Observation: Have clean-up Norvirus kit and the written procedure, but misplaced and in another building. Each food establishment must have procedure for employees to follow when responding to vomiting and diarrheal events within the food establishment to reduce foodborne disease transmission. Moved the Clean-up kit back to school and verbalized understanding that procedure should be accessible to all employees for their view. Corrected on site.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: There is build up around the gasket of "Koche" reach-in cooler in the kitchen. Non-food contact surfaces of equipment must be kept free of an accumulation of dust, dirt, food residue, and other debris. Clean as often as necessary keep clean and prevent contamination. --Observation: Metal shelves under prep sink were rusty. Must be resurfaced, to provide a smooth, cleanable and nonabsorbent surface. Provide the mat or resurface with the light color.
- Drying mops
Observation: All mops were sitting in water in mop buckets. Mops must be air-dried in between uses. Store mop in a manner that will allow for air-drying without soiling walls or equipment.
- Indoor and outdoor surfaces
Observation: Lower section of the wall next to the laundry is peeling away and have a whole. All walls must be easily cleanable and a cove base is required at the floor/wall juncture. Cover whole and make repairs as needed.
- Test kit provided and used to measure sanitizing solution concentration
Observation: Have quad test kits that were expired in 2013 and 2014. Must provide a new test kit for testing the proper concentration of sanitizer solution. Extra test strip took from the Middle school and corrected on site.

Observation: One rack in KOCH four door reach-in cooler is rusting. Must resurface to make smooth and easily cleanable surface.

Observation: Couple deli meat packages were not date marked in reach-in cooler. Must date mark all ready to eat food if not used within 24hrs after opened or preparation.

Four door reach-in cooler next by hand sink was at 54*F for inside temperature. Inadequate cold holding temperature. Removed all foods out from the cooler. Call for service. May not use cooler until approval has been given through health department.

The bottom shelf in walk-in freezer has at less 6" high. Adjust shelf at least 6" off the floor for cleaning purposes.
