Observation: Several food and related items stored on the floor in several areas (hallway in back and front of store, both walk-ins and storage room in the front of the store).

Observation: Return air vents in the kitchen and dining room are full of dust.

Observation: Walls throughout establishment are soiled.
- Outer openings are protected
Observation: Screen door has holes in it and also it doesn't fit the door frame tight.
- Cleaning of maintenance tools and mop water disposal
Observation: Floors in the back hallway, walkin freezer and cooler all very soiled.
