Observation:Wood table used as cutting board is heavily scored, no longer smooth/nonporous/easily cleanable, is also used as a desk

Observation:Observed frozen oysters with no store label, no shellfish certification number/packing date on container
- Material characteristics of non-food contact surfaces
Observation:Observed exposed wood around garbage can in meat dept., not smooth, nonporous easily cleanable.
- Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
Observation: Employees have not been trained in employee health reporting, issued Form 1-B to be signed and dated as proof.

Observation:Observed numerous frozen and refrigerated foods and candies packaged in meat dept. that are not labeled with ingredients and allergens
- Light bulbs, protective shielding
Observation:There is no light cover on lights above produce work area
- Plumbing system maintained in good repair
Observation:This is actually for 5-203.13. There is no mop sink/service sink for disposal of mop water connected to city sewer.
- Indoor and outdoor surfaces
Observation:This is for 6-201.11. Wall covering by produce sinks is peeling, no longer smooth/nonporous and easily cleanable. Walls and ceiling in storerooms are unpainted drywall.
- Dressing areas and lockers
Observation:There are no facilities for storage of employee coats, purses, etc. away from food and equipment. Observed coats and purses in various locations with food.
- Poisonous or toxic materials used and applied
Observation:Observed can of unapproved pesticide on shelf in produce dept. - discarded by employee
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Observation:There is no divider separating raw shell eggs from packaged cheese in grocery display cooler.
- Manual warewashing sinks requirements
Observation:There is only a two-compartment sink in produce, but more than 2 utensils are washed - there is no three-compartment sink with hot and cold running water and drain, and drainboards.
