    Observation: Several ceiling tiles are damaged and one is missing in the back storage. Ceilings must be designed, constructed, and installed so they are smooth and easily cleanable. Replace.

    Observation: No paper towels by handsink. Must provide disposable towels or heated-air hand drying device to every handwashing sink. Restocked now.Corrected on site.COS --Observation: No hand soap at handwashing sink. Each hand sink must be provided with a supply of hand cleaning liquid, powder, or bar soap. Manager moved the soap to restroom and ordered supplies for this one.

    Observation: Half and half coffee creamer were date marked used by 06/12/2015. in coffee creamer cooler. Potentially hazardous food (PHF) prepared and held in food establishment for more than 24 hrs. Must be clearly marked with date made, consume by, or sell by, and be held at a temperature of 41*F or less, for no more than 7 days from day marked. Manager removed and discarded voluntarily. Corrected on site.
