- Posting of a valid license
Observation: Valid food license was not posted and viewable to consumer.
- Food storage - preventing contamination from the premises
Observation: Dozens of boxes of items are stored on the floor in the walk in refrigerator. All items must be stored at least 6" off the floor.
- A temperature measuring device with a suitable small diameter probe is provided and readily accessible to measure the temperature of thin foods
Observation: Staff could not provide a thin diameter, tip sensitive thermometer. A thin diameter thermometer shall be provided for accurate temping of thin foods .
- Food is properly labeled
Observation: Some packaged foods missing food allergen information. All labels must comply with section 3-602.11 on food labeling.
- Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days
Observation: Mustard Mayo in walkin cooler dated 1/7/16 held longer than 7 days. Staff stated mayo and sugar is added. They usually keep for 14 days. And that this was marked improperly as it had just been made on 1/14. Staff updated product date at inspection. 7 days holding time is recommended for future mustard mayo sauce.
- Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
Observation: Some but not all employees had filled out forms 1A and 1B, all employees must have these forms on file and be informed.
- Linens and napkins, use limitations
Observation: Leafy greens in walk in cooler were covered with cloth. Linens, such as cloth napkins, may not be used in contact with food, for storage.
- Test kit provided and used to measure sanitizing solution concentration
Observation: Test kits for sanitizer were either expired or wet, rendering them not usable for testing of sanitizer.
