Observation: Milk stored in a plastic pitcher uncovered with internal temperature 42.5* in a small reach- in cooler. This is a REPEAT violation. Employee stated the lid is broken and they have special pitcher for milk. Removed and correct on site.

Observation: Didn't have a thermometer in refrigerator in warewashing room. Must provide thermometer to ensure adequate temperature is maintained.

Observation: Milk stored in room temperature in dinning room was temped at 55.2*F. Must provide cold holding equipment for milk to ensure the temperature of potentially hazardous food can be maintained at 41*F or below.

Observation: Card board were reused as mat in cabinets. Must provide smooth and cleanable container for this purpose.

Observation: QA sanitizer in sprayer bottle was tested too strong. Must maintain QA sanitizer between 200 to 400 ppm. Corrected at time of inspection.
