Observation:Wooden shelf holding pizza boxes at end of pizza oven area has bare wood showing, reseal with at least two coats of light colored paint or varnish, or cover area with a more durable material to prevent paint chippping/wearing off.

Observation:Has spray chemical on lower prep table rack near pizza boxes and scales, must move to area separate from food service items.
- Responsibility of the person in charge to restrict
Observation:Has employee with bandage over a cut, must use glove on this hand at all time while working with food and clean equipment. Wearing glove now.
- Food on display is protected from contamination by consumers
Observation:Has three open hot wells of food with no lid or sneeze guard on, must protect these from consumer contamination by use of lid/shield or other means.

Has carpeting on storage room floor, remove to make flooring smooth and cleanable surface.

1) Is storing portable pizza racks directly on floor, must store all foods at least 6 inches off floor, will now store on shelving in walk in cooler.

One dented can of saurkraut, unable to be opened on one end, remove from usage now. Corrected on site.

1) Sanitizer bucket at 200 ppm, must dilute to 50-100 ppm of chlorine, diluted now. 2) Has spray chemicals stored next to scales and pizza take out boxes, must keep in area separate from food and clean equipment, moved now. Both corrected on site today.

1) Observed scooping ice and some ice cubes coming in contact with hand 2) Observed touching edge of pizza as transferred to serving pan. Must use barrier method, such as utensil, deli paper, or gloves to keep from having bare hand contact with ready to eat foods. Corrected on site with education.

Observed staff intending to handle clean dishes after prepping soiled items, must wash hands after handling soiled items. Corrected on site.
