    Heavu ice build-up inseveral reach-in freezers. Must defrost adn clean freezer more frequently to prevent food contamination.

    Quat sanitizer tested over 400ppm. Facility using 90 second test strips. not holding strips in solution for 90 seconds. Suggest providing 10 second test strips. Quat sanitzier must be maintained at 200ppm. CORRECTED ON SITE.

    Must wear an effective hair restraint when handling food. Ex. hat, hair net, visor, or bandana. REPEAT VIOLATION.
