Observation: There were boxes of pop syrup sitting on the floor in the backroom.

Observation: The areas behind the pop nozzles and ice chutes of the self serve pop machine were soiled.

Observation: The quaternary sanitizer spray bottle exceeded the required concentration. 200 ppm is the concentration to aim for.
- Storage of clean linens, single-service, and single use articles
Observation: There were single service-ware cups sitting on the floor in the walk-in cooler.
- Food reheated to 165°F or above in a microwave for hot holding within 2 hours
Observation: Food in microwave was not reheat to 165 degrees. Employee used the instructions on the package and did not take temperatures of the product with a thin tipped thermometer.
- Physical facilities maintained clean: Cleaning frequency, restrictions, and methods
Observation: The floor/walls/ceiling in the walk-in cooler were soiled/dusty. The floors in the back room were soiled underneath equipment.
- Separation from food, equipment, utensils, linens, and single service
Observation: There were chemicals placed on a shelf above the three compartment sink.
