Observation: The PIC could not remember the 5 symptoms of food borne illness. I went over his sick policy with him and helped him out.

Observation: The ice buckets were not labled. The person in charge labeled them prior to me leaving.

Observation: The intake vents were not clean to sight.

Observation: Single use containers were not being stored in plastic sleeves or dispenser units.

Observation: The person in charge did not remember the sick policy and could not explain the symptoms of food borne illness. The facility did provide a reporting agreement and they were all signed.

Observation: The hand sink handle and the side of the counter where tape is being used to fix the item is not in good repair.

Observation: The person in charge did not know where to find the sick policy. The person in charge did not know the 5 symptoms of food borne illness and the 5 symptoms of food borne illness. I provided a sick policy (form 1-B) and went over the sick policy with the person in charge. She stated she would place it into the policy if it was not already.
- Handwashing signage
Observation: The hand sink in the prep area did not have a hand wash sign placed by it. I provided a hand wash sign and the person in charge placed it by the sink.
- Sanitizers
Observation: The bleach water was 200 ppm. The person in charge remixed the solution and it was 50-100 ppm.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: The little fan in storage was not clean to sight. Clean the unit prior to placing it in the prep area for the summer.
