A reinspection will be conducted to monitor for compliance.Reviewed with operator that only prepackaged; commercially prepared foods can be sold under this grocery store permit.

Please repair the non-working light in the "true" reach in refrigerator by 03/20/13. All other items corrected.All cold holding units checked OK and < 40 F. Handsink properly stocked and functional. Garbage dumpster is shared with stripmall. Main grocery store is carpeted. Bathroom floor is impervious. Lighting adequate. This grocery store sells only prepackaged foods. Potentially hazardous foods are sold that are stored frozen such as precooked meats; frozen vegetables and ice cream bars. This facility will be assigned a risk category 1 grocery store. No open food products may be sold at this store. OK to operate; OK to issue health permit. Please post official health permit in a conspicuous spot inside store once received via US Postal Service.An annual inspection will be conducted within 90 days. This inspection will be unnannounced.

The following items must be corrected prior to issuance of health permit to operate a grocery store:1) Handsink in restroom must have both hot and cold running water under pressure. Observed no hot water at handsink. This must be corrected by 03/14/13. Failure to comply will result in immediate suspension of health permit. Operator stated she will call to repair handsink.2) Thermometers must be present in all cold holding units including reach in icecream bunker and reach in upright freezer. * All food observed frozen.3) Replace non-working light in "true" refrigerator. 4) Provide a scoop container for the storage of the scoop for bulk food.5) Remove all unused equipment not used or non working in the business. Please call health inspector when hot water is repaired by 03/14/13 at 328-2682 and all other items by 03/20/13 or when ready for reinspection and all items have been corrected above.
