Observed no liquid soap and no sanitary disposable towels at employee restroom and only handsink in grocery store. Keep hand sinks stocked with soap and paper towels at all times to ensure proper hand washing.

Facility must have approved garbage service from Waste Management for any putresible waste. Castaway and any other approved garbage service may be used for non putresible waste such as recycleable items such as cardboard; aluminum; and plastic.Any Washoe County health permit must have garbage service provided by an approved garbage service and cannot be hauled away by owner/operator. This is required to operate your grocery store.

Floors dirty. Clean thoroughly to prevent vermin attraction and maintain sanitary conditions. Grocery store has carpet throughout floor. Floor must be vacuumed daily. Failure to keep floors in a sanitary condition may result in resurfacing the floor with an easily cleanable surface. Please clean the ceiling fan of heavy dust debris buildup.

NOTES:1) Ensure that all potentially hazardous pre-packaged foods are stored in a cooler with a thermometer.2) The carpet is acceptable (tightly woven. clean). If at any time in the future you want to prepare food; the carpet may be required to be removed and replaced with a durable smooth and easily cleanable surface. The carpet must be maintained in a clean condition or it may be required to be removed and replaced with the same or a more durable alternate material.3) There is no mop sink within the facility however the only areas that need cleaning are the restrooms. The rest of the facility is carpet. The floors in the restroom must be cleaned by hand.4) This permit is for prepackaged food only - There is no adequate facilities for cleaning of any kind of scoop or utensils. If selling bulk items that must be scooped or dispensed with tongs they must be disposable single use type.OK TO ISSUE PERMIT TO OPERATE W/ CONDITIONS LISTED ABOVE.
