- During previous inspection, facility was notified about wearing effective hair restraints. The facility did not have any hair restraints to wear during cooking. The manager, Theresa, called Gordon Food Service during inspection to inquire about getting hair nets for facility.
- Observed measuring cups being stored on top of paper on top of prep table. All food contact surfaces must be non-absorbent and easily cleanable. Paper was removed from prep table during inspection.
- Observed broken floor tiles by vent near prep table in kitchen. Floor tiles must be maintained in good repair to ensure proper cleaning. Repair or replace floor tiles.
- Observed black growth inside and around the top of the 3 compartment sink/handwashing sink floor drain. All equipment attached to floor must be cleaned as routinely as necessary.
- Facility had permit on shelf above prep table which was out of view of the public. Manager stated it keeps falling off of its hook. Discussed with manager that the permit must be in conspicuous view of the public. Manger rehung the permit in view of the public.

- Observed manager not wearing hair restraint while prepping pizzas. Always wear a hair restrain (hat, hair net, etc) when prepping foods.
- Observed build up on the sides of the ice machine. Wash, rinse, and sanitize all non-food contact surfaces on a routine basis.
- Observed broken floor tiles around the vent near the back of the ice machine. Repair or replace the tiles.

- The bottom of the prep cooler, the doors seem to be off balance and crooked. When yow close them, they seem to catch in each other and don't close all the way without force. I pointed this out to employees. If the doors aren't closed completely it could raise the temperature.
- The chlorine sanitizer bucket was ~50ppm or lower. It was made with hot water. Explained you should use lukewarm water, never hot. Also suggested using a measuring tool of some kind to be able to make it easier.
- The ice machine drip tray had black buildup. An employee wiped it off.

- Observed employee improperly washing their hands. They touched the handle to turn of the faucet. The papertowels are located in the front of the sink down below where it is not as accessible. It might be beneficial to have a paper towel dispenser that is visible while washing hands. There is also a hand washing sign posted that has the steps. Discussed with manager having a training going over hand washing. After you conduct the training please email me with the details of the training and how many employees attended.
- The sanitizer bucket in the kitchen with chlorine sanitizer was about 10ppm. The manager made another bucket of sanitizer and tested it to be 100ppm.
- In the basement there are both live and dead insects. The mop sink is in the basement so employees do go into it. One live insect was on the stairs. Per manager she was unsure of whether the facility had a license pest management company that inspects the facility. Suggested discussing with owner because there is the potential of insects coming upstairs and getting into the kitchen. I will check back on this to verify if there is a company that will inspect.
- The facility's permit was not posted at time of inspection. Manager said she would contact the owner to see if he has it. Otherwise you are able to get another copy by contacting TCHD.
