  • Observed bulk food item out of its original packaging that was unlabeled. Provide label.
  • Observed food debris accumulation on can opener. Wash, rinse, and sanitize and ensure routine cleaning.

  • Observed sliced ham in zip-loc bag in the walk-in cooler that was dated 6/5/15. Per the manager, the ham had been frozen on that date and had been recently moved from the freezer to defrost in the walk in cooler. I explained to the staff that the item should be labeled with the date that it is removed from the freezer as the item needs to be used or discarded within 7 days. The ham was voluntarily disposed of by staff at time of inspection.
  • Observed dust accumulation on walls and ceiling throughout kitchen especially in the prep room where spices are stored. Clean, and ensure routine cleaning.
  • Facility has temperature documentation that is not filled out completely. Please submit 2 weeks of temperature documentation by 7/10/15. Facility does not have any cool down documentation. Discussed cool down requirements with staff. Emailed a cool down chart to facility. Please submit 2 weeks of cool down documentation to this department by 7/10/15.

  • Observed debris accumulation on the following items/surfaces: Inside the ice machine, Container used to store ice scoop, and the outside of the juice machine. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.

  • There was duct tape on the corner of the cover to the plate warmer. It appeared it was there to keep the cover on. The kitchen manager removed the duct tape. Also the paper towel dispenser was broken. Per kitchen manager he does have a work order in for it. You are still able to access the paper towels.
