- Observed chopped tomatoes being stored at 49 degrees F in prep top cooler. Currently at 10 am, employee stated she put tomatoes in cooler around 4 am or 5 am. Tomatoes were voluntarily discarded by manager. Different sliced tomatoes in same cooler were 41 degrees F. Ice was being used as a cooling medium placed under metal bins. Per manager, this cooler has been malfunctioning on and off for couple months and maintenance has been working on it. Per manager, temperature logs for cooler are taken 3 times a day. Instructed manager to take temperature logs every 4 hours and if food remains above 41 F for that time, then food must be discarded. Re-inspection will be conducted after 10 days of this inspection to ensure cooler is working correctly.
- Observed build-up of food debris inside microwave.Ensure routine cleaning of food contact surfaces.
- Observed debris buildup on the handle bar of microwaves and in gaskets of reach in coolers throughout the facility. Ensure all non-food contact surfaces are routinely washed, rinsed, and sanitized. Per manager, LEEANN NEAVEAR, new gaskets for the coolers have been ordered.

- There was no thermometer provided in the cold storage cooler where the hot dogs are stored. All cold storage units where potentially hazardous foods are stored must be provided with a thermometer. A thermometer was provided at the time of this inspection.
- Observed gasket on glass freezer and on the dipping box freezer that are damaged. Repair.
- Found build-up of grease/debris inside microwave. Clean and ensure routine cleaning.
- Observed debris accumulation on the following items/surfaces: Ice cream freezer, Glass freezer, Container where rolled silverware is stored, Dipping box refrigerator, and the Cold storage well where hot dogs are stored. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Observed dishes that were stacked together wet. Ensure dishes are air dried before storing.
- Observed debris accumulation on walls in kitchen especially near the shake area and near the passover counter. Clean and ensure routine cleaning.

- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to handles, seals, and the bottoms of coolers.
- Observed build up on walls especially behind sinks and equipment such as the grill. Clean the walls.

- Observed drain flies throughout kitchen and managers office. Facility must eliminate flies. Suggest contacting a pest control operator. A follow-up inspection will occur in 10 days for compliance.
- Observed food debris build-up on the floor in the walk-in freezer. Clean floor.
- Observed litter scattered near the back door and drive thru outside meal board. Remove litter and place in dumpster.
