  • Observed soiled knives being stored as clean. Manager voluntarily removed knives from storage and placed in dish machine area to be re-washed, rinsed, and sanitized. Ensure all utensils are washed, rinsed, sanitized, and visibly clean before being stored.
  • Observed heavy debris accumulation in and around microwave near ovens and on microwaves on main cookline. Also observed debris accumulation on grills and vents above grills. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning. Per manager, vents are scheduled to be cleaned.
  • Observed soiled tops and surroundings of prep top cooler and reach in coolers in main cookline. Routinely clean these surfaces.
  • Observed wall/floor junctures in walk in cooler un-attaching from walls. Re-seal or re-attach coving junctures to wall.

  • Observed the following items stored in the walk in cooler dated 12/6. Per the labels on each item, each item had been in the cooler for ~12 hours. Observed one container of chicken gravy at 45.3 F, Once container of chili at 45.3 F, Once container of broccoli soup at 43 F, Bean soup at 44.6 F and 45.3 F, Once container of brown gravy at 47.5 F, Stuffing at 50 F, 55 F, 54 F, 50.75 F and 43.9 F, Once container of vegetable beef soup at 43 F, Chicken noodle soup at 48.9 F, 53 F, 53.5 F, 50.4 F, and 45.1 F, Green beans at 47.1 F, 52.9 F, 51.4 F, and 45.3 F. These items were voluntarily discarded by the manager during this inspection. These items were stacked directly on top of eachother in stacks of 4-5 containers. The lids were not vented. Per the facilities procedures, cool down is to be done on individual racks with vented lids. Ensure proper cool down procedures are followed and provide 10 days of cool down documentation to this department by 12/20/15.
  • Observed food items stored in the cooling units that were not covered. Ensure food is covered while in storage.
  • Observed kitchen employees working without hair restraints. Ensure proper hair restraints are worn.
  • Observed inside of ice machine in disrepair. Repair. Observed rust accumulation on racks in reach in cooler next to the left fryer. Reseal or repaint.
  • Observed wiping towels stored on counter-tops in kitchen. Ensure wiping cloths are stored in sanitizer solution when not in use.
  • Observed heavy debris accumulation in microwaves on grill line, and on the grills and fryers. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
  • Observed heavy debris accumulation on equipment and surfaces throughout kitchen as discussed with manager during inspection. All surfaces, handsinks, racks in walk-in cooler, cooling and hot holding units (especially seals), handles, and equipment must be washed, rinsed, sanitized and cleaned on a regular basis.
  • Observed dishes that were stacked together wet in storage. Ensure dishes are air dried before storing.
  • Observed missing coving beneath window to office near the grill prep line. Replace. Observed cracked and chipped flooring tiles throughout kitchen especially in the dish washing area and on the grill prep line. Replace.
  • Observed debris accumulation on floors throughout facility as discussed with manager during inspection especially beneath equipment and shelving, by the ice machine, and in the walk-in freezer. Clean and ensure routine cleaning.
  • Observed debris accumulation on walls throughout kitchen as discussed with manager during the inspection. Clean walls and ensure routine cleaning.
  • Observed damaged ceiling tile above the fryer in the area where the grills are located. Replace.
  • Observed missing light shield in the dry storage area. Provide shield for light.
  • Staff at facility has not met food handler requirements. Ensure all staff has food handler certifications by 1/7/2016.

  • Observed 2 containers of bulk ingredients not labelled. Manager removed them for cook to place label on them.
  • Observed employees in kitchen/prep area not wearing hair restraints. Per manager, servers do salad prep. Always wear a hair restraint (hat, hairnet) when working with open food.
  • The quat sanitizer in the swab bucket was high ~400ppm. Employee diluted solution to 200-300ppm.
  • Observed baked on food in the oven and food debris in the microwaves near the grill. Wash, rinse, and sanitize on a routine basis throughout the day.
  • All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to counter between equipment, handles, racks, the front of the microwave, and the soda grate.
  • Observed dish machine racks stored on the floor. Store all clean equipment at least 6
  • Observed build up on floors especially in corners and under equipment. Clean the floors.
  • Observed gaps around pipes and cables going into the ceiling. There is a hole in the wall in the walk-in cooler and insulation is showing through. Observed broken/chipped areas of the walls especially on corners. Seal all gaps and repair all holes. Several vents are outside the grid. Replace vents.
  • Observed buildup on walls especially near equipment. Clean the walls.
  • Ceilings vents have a lot of dust build up. Per manager, vents are cleaned once a week. Clean vents and maintain.

  • Found 3 cans of diced tomato's and 1 can of green beans with dents along the seals stored on can rack. Manager removed from can rack and placed in office.
  • Found several uncovered food in the walk-in freezer. Cover foods.
  • Observed sanitizer cloths not fully stored in sanitizer solution. Make sure sanitizer buckets have more solution or provide less cloth towels in buckets.
  • Observed heavy grease and food debris build-up under the grills equipment. Surfaces shall be cleaned everyday. Found various door handle's to kitchen equipment soiled. Observed kitchen prep cooler's door seal gaskets with food debris build-up. Wash, rinse and sanitize surfaces.
  • No hot water provided in both women's and men's restrooms after running metered faucet several times. A follow-up inspection will occur on Feb 6, 2015 for compliance.
  • Observed food and other debris build-up on the floor behind the dish machine and kitchen equipment. Sweep and mop floor.
