- Observed chopped ham with a date label of 12/27/15 in reach in cooler, 17 days since today's inspection. Ham was voluntarily discarded by manager. Also observed container of chopped lettuce in walk in cooler which was unlabeled and sliced oranges in bar's cooler which was not labeled. Both foods were voluntarily discarded by manager. Ensure all cut or open foods are correctly date labeled and are used by or discarded 7 days after that date.
- Observed bag of cheese being stored on same tray as an unsealed bag of raw chicken in reach in cooler. Cheese was moved to higher shelf. Ensure all raw poultry is stored on lowest shelf and ready to eat foods are stored on highest shelf to eliminate likelihood of cross-contamination.
- Observed employee's personal food being stored next to restaurant food in standing cooler and also in walk in cooler. There was a bin labeled
- Observed rusty shelves in 2-door standing cooler. Replace or re-surface shelves to prevent debris from falling off. Observed torn gaskets on standing freezer and in main kitchen reach in cooler. Replace gaskets. Also observed unsealed wood in paper products cabinet shelf in the bar. Paint or otherwise seal all wood surfaces so they are smooth, durable, easily cleanable, and non-absorbent.
- Observed grease and debris buildup on vents above cookline stove. Routinely clean these surfaces to prevent buildup.
- Observed debris buildup in the following areas: insides, front door, and handle bars of ice tea reach in cooler; front door and handles of 2-door standing cooler; and on front door and handles of standing freezer. Routinely wash, rinse, and sanitize these surfaces. Also observed buildup in bar's soda nozzle. Routinely clean this nozzle and the nozzle holder.
- Observed bar's handsink and 3-compartment sink to be leaking. Contact a licensed plumber and repair fixtures.
- Observed rubber mat in bar's handsink. Manager removed item from handsink. Ensure all handsinks are clear and easily accessible to employees. Items sitting in handsinks reduce the likelihood of employees washing their hands.
- Observed missing coving on floor and wall junctures next to fryers. Install missing coving strip.
- In the basement, observed several pieces of old equipment, furniture, and miscellaneous items being stored. Per manager, several of these items have no use for the facility and are put here as an end stop. Instructed manager, any items not necessary for the operation of the facility should be removed from the premise to reduce clutter and pest harborage areas. No evidence of pests in the basement. Facility does have a licensed pest control hired.

- Observed chipping paint on cabinet beneath soda machines. Repaint or reseal to allow for proper cleaning. Observed hole in wall of walk-in refrigerator. Repair hole to allow for proper cleaning.
- Observed a build-up of debris on the following: underneath stove line and on the vent of the ice machine. Clean routinely.
- There was no backflow prevention device observed on the ice machine drain. Provide an air gap or another means of backflow prevention for ice machine by 11/16/15.
- Observed a build up of debris on floor by chemicals. Clean.
- Observed cardboard on floor in kitchen. Cardboard is absorbent, and the floor must be smooth, easily, cleanable, and non-absorbent. Manager removed cardboard from kitchen. Observed seal and paint chipping on floor of walk-in cooler. Repaint, reseal, or repair to allow for proper cleaning.
