- Observed all the items below the second shelf of the standing refrigerator closest to the handwashing sink reading between 45F-49F. The top two shelves were full of trays and food that prevented air flow to the bottom of the cooler. Manager stated he will ensure that air flow is impeded by over packing in the cooler and discarded all items not temping correctly.
- Observed uncovered ice in the waitress areas, bar in outdoor area, and meat in the pizza prep cooler. All foods must be covered when in storage, except when in use, to prevent contamination. Provide covers for all foods.
- Observed moist wiping towel stored in a bucket on a counter without sanitizing solution. All wiping cloths used to wipe food contact surfaces must be stored in sanitizer solution between uses or placed in laundry.
- Observed both sanitizer solutions in the waitress stations to be low. Sanitizing solutions in buckets must be at least twice the strength of that in the three compartment sink. Bartender changed both to proper concentrations of ~300 ppm.
- Observed the three drawer hot holding unit to have visible amount of debris inside. Clean as routinely as necessary.
- Back exit door does not completely close by itself. All doors to teh outside of the facility must be self-closing, tight-fitting to prevent pest entrance. Repair door by 2/24/2016.
- Observed the following damages to the floors that need repair: broken tiles in back room by alcohol cooler, damaged wood floor by the corner of the main bar, build up of debris in a split between tiles and wall by entrance to men's room, and the back bar is need of sealant. Repair to allow for proper cleaning.
- Observed the following areas of the floor with a build-up of debris: under shelving in walk-in freezer, under ice machine, under soda lines, behind cooking equipment, in back room by alcohol cooler, on floor by alcohol on wood, under ice holder in main bar, under three compartment sink in main bar, and the drain in the outdoor bar. Clean as routinely as necessary.
- Observed the following walls/ceilings in need of repair: hole in ceiling above deli slicer, paint peeling on kitchen bathroom door, paint peeling on back exit door in kitchen. Repair to allow for proper cleaning.

- Observed ice uncovered in waitress area. The ice had food debris inside. All food and ice must be covered when not in use to prevent contamination.
- Observed employee grab pickle with bare hand to place on plate. The employee properly washed his hands before hand. Kouri's Pub does not have a bare hand policy, all employees must wear gloves or use a utensil when handling ready-to-eat foods. Discussed with manager and employee.
- Observed the following food contact surfaces in need of repair to ensure proper cleaning: damaged spatulas hanging above 3 compartment sink, knife with damaged handle in knife holder by slicer, paint peeling and rust on dry storage shelves in kitchen, and tape on handle of the beer garden bar beer cooler. Repair or replace to ensure proper cleaning.
- Observed the guide rails of the glass sliding door to have a build up of debris. Wash, rinse, and sanitize as routinely as necessary.
- Observed wet towel in kitchen outside of sanitizer bucket. All wet towels used to wipe food contact surfaces must be placed in laundry or in sanitizer between uses.
- Observed a build up of debris on floor of both bars. Clean routinely.
- The following areas of the floor are in need of repair to ensure proper cleaning: gap in between floor and wall by clean dish rack with build up of debris, damaged floor tiles by back door, beer cooler floor is in need of new seal or painting, and damage in beer garden bar. Repair to allow for proper cleaning.

- Observed taco shells uncovered near ceiling and roast beef uncovered in cold storage near grill. Cover all foods while they are in storage.
- Observed grill cook applying toppings to salad and then sandwich using bare hand contact. Per kitchen manager, the facility does not have a bare hand policy. Employees cannot touch ready to eat food without wearing gloves. Manager spoke to employee at time of inspection.
- Observed two employees wash hands and then touch the handle of the paper towel dispenser with their bare hands. Discussed hand hygiene training with kitchen manager. I will attach a copy of TCHD's updated hand hygiene flyer to the emailed inspection. Manager can use the flyer for hand hygiene training. Have employees sign flyer and mail to me by 05-22-2015.
- Observed food splash in microwave. Wash, rinse, and sanitize on a routine basis.
- Observed build up on the ice machine drip trays. Wash, rinse, and sanitize on a routine basis.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to handles, door seals, and knife handles.
- Observed build up on floors especially in corners and under equipment. Clean the floors.
- Observed gaps in the ceiling around pipes and wiring. Per manager they have been replacing the ceiling tiles. Seal the gaps in the ceiling.
- Observed build up on walls especially in corners and along seals and molding. Wash the walls.

- Found the following dented cans stored with other good cans on the can rack: 2 Chili Sauces, 1 Cheese sauce, 3 Sauerkraut and 1 Refried beans. Employees shall monitor cans before storing them. Cook removed and labeled cans.
- Observed sanitizer cloths not properly stored in sanitizer bucket in the cooks line and the north wait staff station; hanging over the side of the bucket . Submerge in sanitizer solution.
- Observed mounted can opener have food debris build-up; meat slicer unit has food debris build-up. Wash, rinse and sanitize items.
- Wash, rinse and sanitize the following: Observed inside portions of the salad prep cooler handles soiled; 2-door glass cooler salad dressing soiled; pizza prep cooler right door seal gasket soiled. Also, observed mice droppings in bar cabinet.
- Observed drinking glasses stacked together still wet in the wait staff stations. Allow glasses to be air dried.
- Observed food debris build-up throughout cooks line and dish machine. Observed cook's line floor mats soiled. Clean.
- Observed several ceiling tiles stained. Clean or replace tiles.
- Observed one of the lights above the pizza oven without a light shield. Provided shield/cover for light.
