Observed milk that expired 5/6/15. Milk must be dated once opened and used/discarded within 7 days.
Observed ready to eat food stored beneath raw chicken and raw eggs. Ready to eat foods must be stored above raw product to prevent the risk of possible contamination. Eggs and chicken were moved by staff at time of inspection.
Observed wiping cloths throughout kitchen stored on counter tops. Store in sanitizer when not in use.
Observed pots, pans, and dishes that have been stored on counter tops and in 3 compartment sink that were dirty. Per the owner, the dishes have been there since 5/9/15. Dirty dishes must be cleaned every day.
Observed debris accumulation on the following items/surfaces: both microwaves, Chili sauce machine, Counter top and shelving by air conditioner, Container where ice scoops are stored, Dump sink, and the Refrigerator in kitchen. Clean these items/surfaces.
Observed food storage containers stored on counters in attached laundromat. Containers were moved at time of inspection.

Observed debris accumulation on floors in kitchen especially by door leading to basement and near mop sink. Clean.
Ceiling throughout kitchen has water damage and is leaking water. Repair.

Found opened cheese sauce in unplugged Gehls machine at the bar at 90F. Keep this machine plugged in at all times when cheese is in it. It was discarded. Found potentially hazardous food in the hot holding at below 135F. Maintain at 135F or above.
Pizza oven at the bar had a build up food debris. Clean and sanitize it.
Ice machine break shield and glass cooler door tracks in kitchen had a build up of pink debris. Clean and sanitize these areas.
Found mop sink hoses sitting in the bottom of the sink with no back flow prevention devises on the faucets. Provide an air gap and/or back flow prevention devises.
Found carpet under part of the buffet line. Remove it and replace it with proper flooring for food areas.
Found cleaning chemicals stored with pesticides in chemical storage closed. Store pesticides in a separate location. Corrected--pesticides were moved to separate room.
Found an unlabeled spray bottle of a purple chemical. It was discarded.
HACCP plan was removed from the facility by previous cook. They are currently writing a new one. No temperature charts are available. Start recording temperatures daily.
