- Observed unlabeled bucket of sugar in cabinet. All bulk foods unless its identity is unmistakable, must be labeled. Manager provided label during inspection.
- Observed no employees wearing effective hair restraints. All employees handling food must wear effective hair restraints (hats, hairnets) to prevent contamination.
- Observed crack forming on counter to left of three compartment sink along wall. Repair to prevent debris build up and allow for proper cleaning.

- Observed the side door to the playground between dry storage and the kitchen propped open. All doors must be kept closed at all times to prevent pests/vermin from entering the facility. Did not observe any evidence of pests (feces, live/dead insects/vermin, etc.). If you wish to have the door open for extra air flow during hot days, provide a tight fitting, self-closing screened door. Manager closed door at time of inspections.

- Observed employee wash hands and touch the faucet handle with her bare hand to turn it off. She then touched her cell phone and then pulled her sleeves down. Then she put on gloves. Subsequent handwashing was not done for the proper amount of time. When washing hands make sure to turn the faucet off using a single use towel and immediately put on gloves. Do not touch anything else in between. Do a retraining with employees, document what you went over, sign and date. Submit documentation to TCHD by 02-06-2015.
- Test strip initally showed no sanitizer in machine. Ran machine again and still no sanitizer. Sanitizer bottle was found to be empty. Manager replaced bottle and reprimed machine. Ran the machine several more times to get chemical into the line. Machine now reading at 50ppm.
- Found black build up on the reach in milk cooler door seals in the kitchen area. Suggested using sanitizer at the higher end of the concentration limit. Wash, rinse, and sanitize seal.
