  • Observed a whole ham in the walk in. Owner stated it was for personal use. Since this is a category 3 facility only frozen food can be provided to the public. Label personal food.
  • Observed the hand sink in the womens restroom to have a leak in a pipe under the sink. Water is pooling and does not easily drain.
  • Observed a dead mouse in the hallway in front of the womens restroom during the walk through with the owner. The mouse was not there before the walk through. The owner stated the pest and rodent controller had recently been there. A patron of the bar removed the mouse and discarded it in the outside dumpster. Routinely clean the floors, specifically after the pest controller has come.
  • Observed dust accumulation on the walls and ceiling of the walk in cooler. Routinely clean.

  • The top of the counter by the bagged ice machine is breaking away. Reseal top to counter.
  • All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the can crusher and the tops of cans.
  • The frame on the door near the bagged ice machine is crumbling and day light is visible around the frame. There is also a hole in the wall near the utility sink. I did not see any evidence of pests in the kitchen area. Repair the door and the hole by 04-02-2015. Contact me if you repair the door before 04-02-2015 and I will come out for a follow-up.
  • Dust build up on floors especially under equipment. Clean the floors.
  • Observed cobwebs on the ceiling throughout kitchen area. Clean the ceiling.
  • Observed several cans of food and roasters on premises. Only keep necessary articles to a category 3 facility.

  • All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to the can crusher. Keep the can crusher closed in between uses to limit dust buildup.
  • Observed several gouge marks in the floor in the kitchen/prep area. Repair the floors.
  • Build up on the floors. Clean the floors.
  • Observed a hole in the ceiling in the men's restroom. Repair the ceiling.
  • Observed extra equipment such as roasters, fryers, and microwaves that are not to be used on the premises. Observed cans of vegetables and bags of food in the prep area. Remove excess equipment and food from the premises.

  • Numerous cans throughout kitchen that have dust and debris on them. All food products should be clean prior to opening.
  • There is a frozen container of ground beef in the freezer that has been opened and refrozen without a label of when the package was opened. All potentially hazardous food that are opened need to be labeled with a date of when they were opened. Any personal foods need to be labeled as well. As this is a Category III facility, the hamburger needs to be labeled as personal.
  • No thermometer located in cold storage unit. Thermometers must be provided in all cold storage units near the door.
  • Observed the bartender cough several times throughout the inspection into her hand without washing them afterwards. I explained that if she has to cough that she needs to cough into her elbow and not directly onto her hand. Have the manager do a retraining with employees about proper hand hygiene. Submit training paperwork to TCHD by 01-19-2015.
  • Found wiping cloths stored on counter. Store in sanitizer pail between uses.
  • Observed build up in the drink dispenser nozzle. Wash, rinse, and sanitizer nozzle.
  • Observed build up on pizza oven rack. Wash, rinse, and sanitize pizza rack.
  • All non-food contact surfaces need to be washed, rinsed, and sanitized routinely throughout the day. These surfaces include but are not limited to the kitchen can crusher.
  • The sink in the women's bathroom was not draining. Fix the sink.
  • Dumpster lid was open at time of inspection. Always make sure the dumpster lid is closed to prevent pests from getting into the dumpster.
  • Observed a hole in the wall near the back of the kitchen area where the mops and brooms are stored. Daylight can be seen coming through the wall. The hole needs to be fixed to prevent pests from entering the facility.
  • Observed what appeared to be mouse feces on various surfaces throughout the kitchen including the tops of cans. Contact a licensed pest professional and submit an invoice to TCHD by 01-19-2015. All kitchen surfaces will need to be washed, rinsed, and sanitized. Suggest using sticky traps.
  • Missing floor tiles throughout the kitchen area. Fix the floors.
  • Build up on floors especially under equipment. Clean the floors.
  • Build up on walls especially near equipment. Clean the walls.
  • There are roasters and other cooking equipment throughout the kitchen. There are ladles hanging near the coolers as well. Remove unnecessary cooking equipment from the facility.
