- Found moist wiping clothes on counter in kitchen.

- Observed worker cutting an onion with barehand contact. Facility does not have barehand policy. Worker put on gloves. Always wear gloves when prepping food.
- Observed worker improperly wash hands. Worker touched the faucet handle with their barehand to turn it off. Worker put gloves on without washing hands first. I went over proper hand washing techniques on site and observed workers exhibit proper hand hygiene.
- Observed worker not wear a proper hair restraint when I first entered the kitchen. Worker put on a hat.
- Observed build up on soda nozzle dispenser behind bar. Wash, rinse, and sanitize daily.
- Observed food debris in the microwave. Wash, rinse, and sanitized on a routine basis throughout the day.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to handles, seals, cabinets, outside of the microwave, and the bottom of the stand up hot holding unit.
- Did not find any paper towels or soap at hand sink behind bar. Always provide single use towels and hand soap to all hand sinks.
- Observed a gap around the large wooden door between the kitchen and bar area. Provide stripping to fill the gap and prevent pest entrance by 07-30-2015.
- Observed cracked floor tiles throughout. Replace tiles.
- Observed peeling paint and holes in the walls throughout. Repaint the walls and seal all holes. Some of the coving along the floor-wall juncture is coming away from the wall. Seal coving to the walls.
- Observed stained ceiling tiles throughout. Replace tiles.

- All food contact surfaces used for cooking need to be washed, rinsed, and sanitized on a daily basis. These surfaces include but are not limited to the inside of the microwave in the kitchen and the pizza oven in the bar area.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to handles, cabinet faces, the bottom of the reach in coolers/freezers, and the top tray on the ice machine.
- Observed cracked floor tiles throughout the kitchen and bar area. Replace floor tiles.
- Observed build up on floors especially around equipment. Clean the floors.
- The seal on the handsink in the kitchen is cracked and broken. Reseal sink to the wall.
- Observed food splash on the walls. Clean the walls.
- Only food manager on premises. Spoke to new facility manager about food handler license. I will send a copy of TCHD's 2015 food handler class schedule with the emailed inspection.

- Found 3 dented cans in dry storage. Manager removed the cans. Monitor cans before they are put on the shelf to ensure there are no dents.
- Racks in the walk in cooler are rusting. The door seal on the beer cooler in the bar area is broken off from the door. Replace.
- Dust debris in the ice machine drip tray. Clean the tray.
- Food splash inside the microwave. Clean the microwave.
- All non-food contact surfaces need to be washed, rinsed, and sanitized on a routine basis. These surfaces include but are not limited to handles throughout the kitchen, the microwave face, cooler seals, and the bottom of the reach in freezer.
- Food and dust build up on the floors especially under equipment and the 3-compartment sink. Clean the floors.
- Ceiling tile missing above the stove. Replace the tile.
- Stains on the wall behind the stove/grill and the 3-compartment sink. Clean the walls.
- Manager did not know where temperature charts were at time of inspection. Mail a week's worth of temperature charts by 12-22-2014.

- In the walk in cooler there was raw pork stored over coleslaw. The coleslaw was moved to the rack on the other side.
- There was a hot holding unit stored under a cork board with tacks. The cork board was removed.
- The slicer was covered in a black garbage bag. It was removed and replaced with a food grade bag.
- The pizza ovens behind the bar had a lot of buildup and cheese on the tray at the bottoms.
- Found no paper towels at handsink at the bar. They were replaced.
- Food damaged and cracked floor tiles throughout kitchen.
- There is a missing ceiling tile and the tiles adjacent to it are stained.
- The lights over the 3-compartment sink do not have the end caps on the tubes.
- The facility did not have temp charts at time of inspection. Discussed taking temps of coolers and freezers before cooking and taking a final cook temp of the first batch. Email me a weeks worth of documented temps.
