  • Observed unlabeled yeast and unlabeled apple cider vinegar in the kitchen. Unless its' identity is unmistakable, all foods removed from original container must be labeled with common name to prevent misuse. Employee labeled the foods at time of inspection.
  • Observed cole slaw dated 1/29 in the refrigerator. All potentially hazardous foods must be used by or discarded by 7 days after preparation. Employee discarded cole slaw at time of inspection.
  • Observed potato skin in true freezer that was uncovered. All foods must be covered or otherwise protected from contamination during storage.
  • Observed the following food contact surfaces in need of repair: seal hand sink in bathroom to wall, fix the coffin freezer in the bar's handle, and repaint peeling white shelves in bar area. Repair or replace to allow for proper cleaning.
  • Observed the following non-food contact surfaces in need of repair: peeling paint and rust on top of coffin freezer and grease trap. Repair, replace, paint, or seal to allow for proper cleaning.
  • Observed large build up of debris inside the Southern Pride smoker. Clean routinely. Observed large amount of ice in freezer in back room. Defrost and clean.
  • Observed build up of debris on the deep fryer. Clean and sanitize as routinely as necessary.
  • Observed build up of debris on floor in kitchen underneath and around equipment, in the bar, and in the secondary bar. Clean as routinely as necessary.
  • Observed the following floors in need of repair: missing tiles in soda room, cracked tiles in bar area, and the floor in the back mop room. Repair or replace to allow for proper cleaning.
  • Observed large build up of debris on the ventilation baffles above stoves in kitchen. These must be cleaned as routinely as necessary.
  • Observed the following areas of the walls/ceilings in need of repair: missing ceiling tiles by soda, wall in laundry area, and peeling paint in men's bathroom. Repair or replace to allow for proper cleaning.
  • Observed personal items stored on the soda line racks and the soda boxes. Enough facilities must be provided for employees to properly store their items away from food and food preparation areas.
  • Observed large amount of equipment, debris, and various other items throughout back storage area of new addition. These areas must remain clean and organized to prevent pest harborage areas.
  • Observed the secondary bar to be disorganized and unclean. Per employee, this bar has not been used for months. All areas of food service establishment must be maintained in good repair and clean, despite use.
  • Observed refrigerator in kitchen that is not in use. Per employee, it was supposed to be removed. All items not necessary to the operation of the facility must be removed.
  • The HACCP plan was not at the facility during inspection. According to the Tazewell County Ordinance, all Category I facilities must have their HACCP plan at the facility at all times. Provide facility with HACCP plan by 2/19/16.
  • The temperature documentation charts for the facility were on the laptop of the main cook. Temperature documentation charts must be on site at all times for employee use. Provide TCHD with copies of current temperature documentation charts immediately and provide a week worth of new temperature charts to TCHD by 2/19/16.

  • Observed an uncovered container of red drink in the 2nd bar beer cooler. Cover food and beverages to prevent contamination.
  • Paint the bare edge of the shelf over the grease trap. Paint bare wood blocks under the shelves in the walk in cooler.
  • The stainless island drawer and shelf in the kitchen, inside of the True freezer in the kitchen, inside of the Keg cooler at the main bar, soda lines, Monster fridge, inside of the True beer cooler at the second bar had a build up of debris. Clean and sanitize these areas.
  • The inside of the smoker had a build up of debris and grease. Clean and sanitize it.
  • The plumbing pipes at the bar and in the kitchen, 3-compartment sink at the second bar, outside of the True freezer (door handles too), and utility sink in the utility room by the second bar had a build up of debris. Clean and sanitize them.
  • Observed boxes of single service cups and plates stored on the floor in the paper product storage room. Store them at least 6 inches off the floor.
  • Repair the leak at the swing arm of the 3-compartment sink faucet in the kitchen and seal the mop sink faucet to the base in the back room.
  • Remove carpet from the second bar floor.
  • Clean main bar floor under and around equipment.
  • Remove dust from the light in the soda room and ceiling throughout the establishment.
  • Paint bare wood ceiling in the soda room and replace missing ceiling tile. Cap the holes in the walk in cooler walls. Per manager, the caps have been ordered.
  • Observed a container of RAID stored with liquid soap inside the white cabinet on the North wall of main bar. Corrected--The pesticide was moved into the office.
  • Remove all unnecessary items from the back storage rooms.

  • The Coca Cola fridge at the bar was at 48F. Maintain at 41F or below. Repair this unit. Continue to ensure that potentially hazardous food is not stored in this unit until it has been repaired.
  • Repair the dripping faucets at both the kitchen and bar 3-compartment sinks and under the hand sink at the bar.
  • Replace the missing or damaged floor tiles in the soda storage room in the kitchen, near the south wall in the dining area, and in the bar area.
  • Observed an unfinished wood ceiling in the soda storage room. Paint or seal it. Repair the hole in the wall above the white chest freezer in the kitchen, Holes in the ceiling tiles around the pipes above the pizza oven in the kitchen, holes in the walk in cooler walls and ceilings, hole in the south wall leading to Th Leeway, hole in the wall across from the back door of the facility, holes underneath the handsink in the bar area, gap between the south wall and The Leeway in the dining area, holes near the door handle near the back door of the facility, and the holes in the back storage area walls.
  • Replace the damaged light shield above the stainless freezer in the kitchen.

  • Observed sliced tomatoes in reach-in cooler next to front kitchen doorway, sliced tomatoes in sandwich make cooler without date labeling, and pizza sauce in a pain in cooler next to soda room without date labeling. Must be labeled and used within 7 days. Manager discarded these items.
  • Found chicken wings, deli turkey, ham, beef, sliced tomatoes, cheese in the sandwich make table at 46-60F. Found orange juice in the coca cola refrigerator at 50F. Cold foods must be held at 41F or below. All of these items were voluntarily discarded by manager. Do not use this equipment until it is repaired and maintaining temperature. Owner was able to correct temperature issue in coca cooler refrigerator in bar area.
  • Observed raw meats in individual pans thawing on top of white chest freezer. All these items were still 41F or below and were immediately moved to a walk-in cooler.
  • Observed unlabeled personal items in the wait staff refrigerator. All personal items must be labeled.
  • Observed drinks in unhygienic beverage containers in the kitchen and in the bar. A container with a lid and straw must be used due to the potential of contaminating hands.
  • Found plastic shelf between fryers in the kitchen was cracked and melted, making it not easily cleanable. Replace with shelf that easily cleanable.
  • Observed sanitizer in bucket in bar area did not have any concentration of chlorine. Buckets must contain chlorine concentration of 100-200 ppm.
  • Found towels stored behind toilet in main women's restroom. Towels should be stored in sanitizer or be laundered between use.
  • Observed the following with a build up of food debris: inside of True Freezer, dough mixer, can opener blade, whisk on prep table, inside of pizza prep table, pizza racks. Clean and sanitize these areas.
  • Observed pizza ovens in bar and kitchen had build up of debris. Clean and sanitize.
  • Observed outside of dough mixer, outside of freezer next to the fryer, outside of the food storage bins under dough roller, service sink near laundry all had build up of debris. Clean and sanitize.
  • Observed the following areas of plumbing with leaks: the faucet in 3 compartment sink in kitchen, faucet in three compartment sink in bar and under the hand sink in bar area. Repair.
  • Observed hose in service sink near laundry without backflow prevention device on faucet. Detach hose, provide backflow prevention or air gap to this faucet.
  • Observed open dumpster lid outside. Manager corrected this on site.
  • The following areas had missing or damaged tiling: the soda room in kitchen, near the south wall in the dining area, and in the bar. Repair or replace.
  • Observed the following areas to have build up of debris: behind equipment in the kitchen, under the equipment in bar, and in the back storage rooms. Clean routinely.
  • Observed unfinished wood ceiling in the storage room, hole in wall near the white chest freezer in kitchen, holes in ceiling above pizza oven around pipes, holes in walk in cooler, hole in south wall connecting to Leeway, hole in wall across from back door of facility, holes in wall underneath handsink in bar area, gap between south wall and dining area bar, holes near back door of facility, and holes near door handle of door entering back room. Repair all damaged walls and ceilings.
  • Replace broken light shielding in kitchen above stainless steel refrigerator.
  • Observed taco meat in cooler that was cooked and cooled down. As a Category II facility you can not cook and cool down. If you wish to change categories, contact the Health Department or discontinue this process.

  • Found 2 half gallon containers of milk and 1 half gallon of orange juice in the Coca Cola fridge in the bar area at 48F. Also, found sausage, pizza sauce, olives, cheese, bacon, and other pizza toppings in the top of the pizza make table cooler at 49-50F. Maintain cold food at 41F or below. Do not use these units to store potentially hazardous foods until they have been repaired and can maintain food at 41F or below. Contact TCHD for a re-inspection when they have been fixed. All of the above foods were voluntarily discarded.
  • Found the following food in the steam wells out of temperature: cheese sauce 109F, pizza sauce at 105F, and Au Jus at 112 F. Maintain food in hot holding at 135F or above. These foods were voluntarily discarded.
  • Found unlabeled Italian beef, sliced and dice tomatoes, and sliced ham in the coolers in the kitchen. They must be labeled and used within 7 days. Also, found Italian beef labeled 6/20 and 6/23 in the sandwich make table.

  • Observed raw hamburger on a shelve in the walk in cooler above pickles and a box of corn on the cob. Store raw meat on lower shelf. Corrected--hamburger was moved to lower shelf.
  • Found a large box of crackers stored on the floor in the waitstaff area by the bar. Store food at least 6 inches off the floor.
  • Paint or replace rusty shelves in the walk in cooler. Replace broken and melted plastic cart between the fryers in the kitchen with a more durable one.
  • Found no chlorine sanitizer pail at bar and no sanitizer in the wiping pail in the kitchen. Maintain chlorine sanitizer at 100-200 ppm. Corrected--new pails set up at 100 ppm.
  • Found dirty towel sitting on counter at bar. Store in sanitizer between uses. Corrected.
  • The following food contact surfaces were soiled: Insides of all coolers and freezers, ice machine interior, soda dispensing nozzles, shelves and counters in kitchen and bar, inside of the walk in cooler(including walls, ceiling, and fan), pizza rack, dough roller, dough mixer, slicer, pizza prep cooler and clod holding unit. Thoroughly clean and sanitizer these areas.
  • The pizza oven at the bar, pizza oven in the kitchen, fryers, grill had a build up grease and debris. Thoroughly clean and sanitize these areas.
  • Found soiled, crusty scoop in the pizza sauce on the pizza make table. These utensils must be washed, rinsed, and sanitized at least every 4 hours. Per cook, these utensils are only washed once per week.
  • The cooler door seals, outsides or coolers and freezers, outside of the walk in cooler, outsides of cooking equipment and attached pipes, water heater, pipes in the bar and kitchen areas, pipes in the washing area had a build up of debris. Thoroughly clean and sanitize these areas.
  • Found several soiled pizza boxes inside the bottom of the pizza make table cooler. Dispose of them.
  • The bar hand sink faucet and drain pipe, the bar 3-compartment faucet and drain pipe, the kitchen 3-compartment hand sink faucets were leaking. Repair all these leaks.
  • The toilet in the back dining room bath room was dirty. Clean the toilet.
  • The dumpster lids were open. Keep them closed.
  • The front doors were propped open. Keep doors closed to prevent the entry of pests. Bartender closed doors.
  • Found build-up of debris on floors, especially under, around, and behind shelves, equipment, coolers, and under sinks throughout the establishment. Clean the floors. Make sure to clean floors in restrooms and walk in cooler as well.
  • Replace missing base coving under hand sink and 3-compartment sink in the bar, south wall of dining room, under the 3-compartment sink in the kitchen, by the water heater, and under the dish drying table in the kitchen. Replace missing floor tiles in the soda box storage area in the kitchen.
  • Found build-up on walls in bar area and kitchen area and pipes in the kitchen and bar areas. Clean the walls and pipes.
  • Repair the holes in the wall by the pass thru window in the kitchen to the neighboring bar, wall by the back door, wall under the hand sink and ice bin in the bar area. Caulk or seal the gap at the wall/counter juncture on the south wall in the dining area. Replace the missing ceiling tile in the dry storage room.
  • Repair the lights that are not working in the dry storage rrom.
  • There was a lot of unused equipment and items and boxes and debris stored throughout this facility and in room by back door. Remove unnecessary items from the facility.
  • Observed wet, dirty mops on floor and in the bottom of the mop bucket in kitchen by the mop sink. Clean mops regularly and hang them up to dry.
  • There was no available certified food manager certificate (FSSMC) posted during the inspection. Send in a copy of the certified food manager certificate to TCHD, Attn:Kim or have one of the cooks sign up to take this training in 30 days.
