  • Observed employee drinks stored on shelving with food contact surfaces. Employee must store personal lidded drinks, with straws, in a manner that prevents contamination of food and/or food contact surfaces.
  • Observed multiple employees with rings on during inspection. All employees must remove jewelry when handling food. Only plain band rings and medical identification jewelry is permitted in food service. All employees removed rings during inspection.
  • Observed a brown liquid in the hand sink located next to the prep table. Per manager, she had just washed, rinsed, and sanitized it, so an employee had just dumped liquid down the drain. All hand sinks must only be used for hand washing.
  • Observed employee wash hands correctly and then turn faucet handle with bare hand. Employees must turn faucet off without using bare hand contact (paper towels, foot pedal shut offs). Employee correctly washed her hands after training. Conduct hand washing training to ensure all employees are correctly doing so and provide TCHD with signed training documentation by 3/11/16.
  • Observed multiple employees with long hair not effectively restrained. The hair was freely able to sway around shoulders. All employees shall wear effective hair restraint while handling food. All employees properly restrained hair during inspection.
  • Observed cutting board of prep table to have visible abrasions and discoloration. All food contact surfaces should be maintained in good repair, free of abrasions, and easily cleanable. Repair or replace.
  • Observed large amount of storage equipment with sticker remnants. Ensure that employees are scraping and soaking all equipment before wash, rinse, and sanitizing.
  • Observed both dumpsters open upon arrival to inspection. When I had walked close to the dumpster area, a squirrel jolted out of the dumpster and away. The facility must ensure that outdoor dumpsters remain closed at all time to prevent pest entrance/attraction.
  • Observed a build up of debris on the floor of the walk-in cooler, especially in the seams, in front of the walk-in cooler door, under the ice machine, and throughout the dishwashing area. Clean as routinely as necessary.
  • Observed a hole and exposed wood directly above the women's bathroom door handle. Doors must be maintained in good repair. Repair to allow proper cleaning.
  • Observed large hole in the wall/ceiling above the chemicals in the chemical room. All walls and ceilings must be maintained in good repair. Repair to allow for proper cleaning and prevent entrance into the facility from the ceiling.

  • Observed some employees without effective hair restraints. All employees handling food should be wearing hair restraints such as a hat or a hair net.
  • Observed a hand sink in the kitchen near the 3 compartment sink without paper towels. The manager provided paper towels during the inspection.
  • Observed the floor by the kitchen exit door and the walk in cooler to have debris build up. Clean these areas.

  • Observed employee cleaning food contact surfaces in kitchen/prep area without wearing hair restraint. Wear hair restraints at all times when working in prep area.
  • Observed cutting board with deep cuts. Refer to manufacturer's manual for reconditioning cutting board or replace.
  • Observed build up on floors especially under equipment and observed food debris on floor in walk in freezer. Clean the floors.

  • One employee had 1 glove on, was taking money, then making coffee all without changing the glove or washing her hands. Another had her hands tucked into her sweatshirt while making coffee and kept pulling her sleeves down over her hands. Multiple employees were taking money then making coffee or getting donuts. Any employees with nail polish or acrylic nails must wear gloves. Also saw a couple of employees with tongue rings that were playing with them in their mouths. If gloves are to be worn they must be changed when changing tasks and properly wash hands before putting on new gloves. I didn't see anyone wash their hands while at the facility. Employees can wear sweatshirts just make sure that the sleeves stay clean and do not contaminate hands. Employees are able to have tongue rings but anytime they open their mouths and are messing with them spit can leave their mouth getting on food and food prep surfaces the same way when an employee is chewing gum.
  • The reach in cooler with milk on the same side as the prep cooler has a broken door seal.
  • There is sticky buildup on surfaces throughout the facility including prep tables, shelving, racks, many of the door seals on the reach in coolers, handles, and the front of ovens.
  • Observed gaps in the bottom of the back door to outside. Door must be repaired so no daylight is visible. I believe this is one way the leaves are coming into the facility. If there are gaps that means pests can enter. Contact me when there is no longer any daylight showing. This must be done by 10-15-14.
  • There are cracked floor tiles by the back door.
  • There is a black buildup on floors throughout facility. Behind prep areas there is frosting and sprinkles as well on the floor. Also there are leaves on the floor especially by the back door and 3-compartment sink area.
  • There is what appears to be frosting splattered on the walls and ceiling in prep areas.
  • There is a sweatshirt stored on the single service rack. Employee belongings should be stored away from open single service and food.
  • There is an unlabeled bottle on the prep table with frosting by the walk in cooler. The bottle was later labeled.
  • There was a container of hand soap stored with the single service items and over coffee beans. The soap was moved to the chemical storage closet.
