- Observed dark liquid stain on the bottom of the #4 cooler in basement. Wash, rinse, and sanitize inside of cooler.
- Store all dishware with the handle or non-food contact surface presented to the handler.
- Observed missing tiles and holes in the floor in dry storage and in kitchen. Repair the floors.
- Observed breaks in the walls and holes in the ceiling. Seal all holes and repair breaks.

- FOUND CHLOR SANT IN WIPING PAIL >20PPM. SANT WAS DILUTED TO 100PPM.
- FOUND LARGE CONTAINER OF COOKED CHICKEN 78* IN REACHIN REFR. FOOD MGR SAID IT WAS COOKED AT 10:30AM TODAY. CHICKEN WAS REHEATED TO 165*. CAT II FOOD FACILITIES CAN NOT COOLDOWN, REHEAT & RESERVE PH FOODS.
