- On the shelves by the hand sink there was crab as well as bagged shrimp in water that were both thawing. The crab was put in the cooler. The owner spoke with employees about the shrimp.
- In the walk in cooler there was raw meat over cheese. The owner had an employee move it.
- Observed food on the floor of the walk in cooler.
- Throughout the kitchen in prep areas I found an open can of soda, an open cup of soda, chocolate milk in a cup, a bottle of Gatorade. Also in a prep area I found a half eaten roll as if someone was eating while cooking.

- Observed the dish washer handle dirty dishes, wipe his hands on a dirty rag, then handle clean dishes without washing his hands. Observed another employee improperly wash his hands. Another employee handled his cell phone then did food prep. Discussed with owner conducting a training not only going over how to properly wash hands, but also when to wash hands as well as making a cell phone policy. I gave him a hand washing hand out. I will need documentation of training.
- There are several shelves and storage racks throughout the facility and those in coolers that are rusty like the produce cooler. Also there is wood utilized underneath foods in the back storage area and other areas in the facility. There is also cardboard used on shelves such as under containers of foods near the fryers. The meat shredder has what appears to be some sort of covering/sticker on the stainless steel coming off. There are rubber bands stacked up on shelves in the prep area.
- Found wiping clothes stored on counters throughout the facility. Store in sanitizer pail between uses.
- Several surfaces throughout the facility have buildup and food debris on them such as kitchen shelves, food container lids, equipment, fan grill covers & ice machine, etc.
- Observed clean dishes being stacked together wet.
- The door in the back of the kitchen was propped open. Per owner sometimes employees prop it open when it gets warm in the kitchen. Discussed not cracking the door open as it allows pests to potentially enter the facility.
- In the dish machine area the flooring is not smooth and there is water collecting in several places. Also it appeared as if the floor drain may not be draining properly as there was an excessive amount of water on the floor.
- There are jackets and coats throughout the facility in prep areas and on food. There were also personal items like backpacks in food areas.
- I found a bottle of medication stored near food on a food contact surfaces in a prep area. The medication was removed. Discussed chemical/medication storage.
- There were spray bottles of chemicals such as Raid and Rain X stored on top of electrical boxes which are over foods. Also on the shelving unit by cans there were 2 cans of paint/wood sealant stored with food. They were removed.
- There are extraneous items throughout the facility such as old equipment. There is hardly any room for storage because there is so much stuff. Discussed going through the facility as if you've never been here before. Also marking items you haven't used in a year then decide if it can be removed from the facility.

- Found temps of raw steak & raw sausage 46* in one reachin cooler. Found temp of Turkey & ham in prep cooler by grill 47*. Found temp of boiled eggs in salad bar cooler 46*. Food Mgr vol discarded PH foods out of temp. Stahl Mechanical repairmen were already there repairing Sushi prep cooler which was also out of temp.
- No thermometer in lower reachin cooler by front grill.
- Found employee getting ice out of ice machine with large pan with no handle.
- Two kitchen employees did not have on beard cover/restrains.
- Found broken rubber spatula used at front grill.
- Found peeled plastic covering on metal frame panels of some kitchen walls/equipment.
- Found several moist wiping clothes stored on counters,equipment.
- Found grease scum/soil/food debris buildup on kitchen shelves, food container lids, equipment, fan grill covers & ice machine grill cover, etc. Also, found rust buildup on metal shelves inside walkin cooler.

- Condenser unit fan in walkin cooler was spraying water onto cooler walls & food containers below. Also the door & door seal to walkin freezer needs repaired. Also, the kitchen employees restroom door handle is broke. Also, found bare wood board used to mount kitchen equipment on.
- Found lent/dust/scum buildup on some shelves, food containers, fan grills, equipment in kitchen. Also, found rust buildup on some shelves & on ceiling grill in walkin coolers.
- Found clean wet metal pans stacked together still wet.
- Found open boxes of single use food containers stored on floor in dry goods area.
- The door handles are missing on employees restroom.
- No paper towels at handwash sink by front grill line.
- No lid on waste bin in employees restroom.
- Floor by dishwasher needs repaired.
