- Observed edge of white counter near stove that has peeled off. Replace. Observed metal rack near stove where trays are stored that is rusting. Reseal or replace.
- Observed debris accumulation on the following items/surfaces: White freezer across from stove, All white cabinets in kitchen as discussed with manager during inspection, Stand-up cooler in bar. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Observed unlidded trash receptacle in restroom that is located in the dining room. Provide lid.
- Observed approximately 40 flies in kitchen. Discussed pest management with staff and recommended contacting a licensed pest remediation contractor.
- Observed cracked floor tiles in kitchen of facility as discussed with manager at the time of the inspection.
- Observed debris accumulation on floors throughout kitchen as discussed with manager at the time of this inspection. Clean and ensure routine cleaning.
- Observed 3 packages of Brats wrapped in foil in the kitchen refrigerator that were labeled COOKED BRATS. Category II facilities may not cool down food items and reheat or reserve. These 3 packages of brats were voluntarily disposed of at the time of this inspection.

- There was no thermometer in freezer in kitchen. Provide thermometer.
- All non-food contact surfaces need to be washed, rinsed, and sanitized. These include handles on coolers, all drawers, freezers, refrigerators, ovens, inside and outside of all cabinets, handsink, and stoves.
- Found several storage containers right side up in cabinets and on shelves. Dishes and storage containers need to be stored upside down.
- Found several cracked floor tiles throughout the kitchen. Replace or repair these tiles.
- Observed food accumulation and debris buildup on floors. Per manager, floor will be getting power washed in the next 2 weeks. Clean and ensure routine cleaning of floors.
- Observed chemical spray bottle without label. All chemicals must be labeled. Contents of bottle was dumped out by manager at time of inspection.
