- Found dirt/lent buildup on floor circulating fan blades & grill in kitchen.
- Some floor tiles are broken/missing in walkin cooler.

- Observed one container of taco meat that was labeled 10/31/15. Ready to eat potentially hazardous food must be used or discarded within 7 days after opening. The taco meat was voluntarily discarded at the time of this inspection.
- Observed debris accumulation in ice machine. Clean and ensure routine cleaning.
- Observed chipped floor tiles near the oven in the kitchen. Repair or replace.
- Observed ceiling tiles in the dry storage area that are chipping and peeling. Replace.
- Observed debris accumulation on ceiling throughout the kitchen as discussed with kitchen manager during this inspection. Clean and ensure routine cleaning.
- Observed two chemical containers that were not labeled. All chemicals must be distinctly labeled. These two containers were labeled at the time of this inspection.
- Observed multiple extraneous and decor items in back storage room. While this is a storage area, be sure to limit to items only for this facility as well as only items that are necessary. Shelving is cluttered and jam packed, this makes cleaning difficult and gives pests a place to hide.

- Found boxes of food on floor of walk in freezer. Ensure all food is stored a minimum of 6
- Observed leaking walk in freezer. Repair or replace. Observed chipped counter edge in kitchen. Repair or replace.
- Found a accumulation of debris on the following items/surfaces: All cooling units in facility (inside and outside), shelving in walk in cooler, Microwave, Shelving beneath grill.
- Found utensils stored with handles stored down. Store clean utensils so handles are facing up.
- Repairman was called during inspection and he would be coming to check the sink today.
- Observed accumulation of debris on ceiling and walls in walk in cooler. Clean. Noticed debris accumulated on ceilings near vents in kitchen. Clean. Observed debris accumulation near white fridge in kitchen and near the handsink. Clean.
- Observed multiple extraneous and decor items in back storage room. While this is a storage area, be sure to limit to items only for this facility as well as only items that are necessary. Shelving is cluttered and jam packed, this makes cleaning difficult and gives pests a place to hide.
- Observed food cooled down at facility. Category 2 not authorized to cool down. Food was voluntarily disposed of by manager at time of inspection.

- The door seal to the upright refrigerator/freezer unit in the kitchen is broken. Also there is a towel on the handle of the pizza oven. The side of the counter in the kitchen by the crock pot is starting to miss the cover. I pointed this out to the employee. All surfaces must be easily cleanable.
- Observed gap around the screen door to outside in the back storage area. The lower left and the top has a noticible gap that is allowing flies to enter the facility. Either the door must be repaired so no daylight is visible or keep the inner door closed. Per employee they will keep the inner door closed so there isn't a gap.
- The floor is cracked and missing pieces in front of the 3-compartment sink.
- There is buildup on floors behind the bar.
- Observed multiple extraneous and decor items in back storage room. While this is a storage area, be sure to limit to items only for this facility as well as only items that are necessary. Shelving is cluttered and jam packed, this makes cleaning difficult and gives pests a place to hide.
