  • 229.167 (p) (11) (C)insects/pests not minimized. Observed live small roaches in the dry storage room and also observed a roach in the kitchen area. Observed roach debris in the food staorage area as well. Minimize the presence of pest by throughly cleaning and using professional pest control.
  • 229.165 (k) (14) (A)chlorine soltion. Clorine solution in 3 compartment sink was reading above 100 PPM. Chlorine solution should be between 50-100 PPM.
  • 229.165 (m) (1) (A)not clean. Shelves in most coolers where food is stored are not clean and food debris was seen on the sheleves. Ensure that the shelves in all coolers are clean.
  • 229.165 (m) (1) (B)grease and soil accumulation. Heavy grease accumulation is in between and underneath food equipment. Ensure that thorough cleaning is done in between and underneath all food equipment.
  • 229.166 (f) (4) air gap 2x. Air gaps were not seen at the ice machine, steam machine, and pip that comes from the hand sink. Air gap between water supply and plumbing fixture must at least 1 inch. Diameter of air gap between water supply and plumbing fixture must be at least twice the diameter of the water supply inlet.
