    Observes several freezer bags of tamales.  Worker and owner stated that the tamales were made in a ladies home (mother of worker).
    Usage of home prepared or hermetically sealed food which has been processed in a place other than a wholesale food processing establishment is prohibited.  All tamales were discarded by owner.  Corrected.

    Observed unlabeled food items in pans in reach in cooler: chicken,  beef, pork.  Label all potentially hazardous foods with name, date and time of preparation.
  • Critical: Hands washed, good hygienic practices (observed).
    Observed owner entering facility and preparing foods without washing hands, observed worker with long painted fingernails handle money and assist in preparing foods without washing hands or donning gloves.
    Hands shall be washed thoroughly before using utensils, including: deli tissues, spatulas, tongs or single-use gloves.
    A food service employee shall not wear fingernail polish or artificial fingernails when working with exposed food unless wearing intact single-use gloves.  When informed, workers washed hands and put on gloves. Corrected

    No sanitizer solution made for sanitizing surfaces or utensils.  Maintain sanitizing solution concentration in bucket at 150-200ppm, sanitizing solution in 3 compartment sink at 50-100ppm.  Worker made fresh sanitizing solution in bucket at 200ppm, in 3 bay sink at 50ppm.  Corrected.
  • Food contact surfaces of equipment and utensils clean
    Upper interior of microwave soiled. Clean and sanitize food contact surfaces.

    Right spigot on 3 bay sink leaks.

    Both spigots at 3 bay sink leaking.
    Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.

    Observed no trash cans for restrooms and handsinks in food prep area.  Provide trash cans with lids for appropriate areas.
  • No Certified Manager Present
    There currently is no certified person on file for this facility.
    As a Category I facilty it is a requirement that a state certified person be present at all times food is being prepared.  Please provide a certified person by.1/18/2012.

The following was discussed with Kelly Castillo, the owner:

All exterior doors need to be self closing.(Front door)

Kelly stated the lights do have light shields, but they are not installed at this time.

There are two hand sinks which have the drain pipe above the bottom shelf. There shall be not food or food items stored under neat the drain pipes. A trash can, wiping cloth bucket or chemicals can be stored below the drain pipes.

The 3-compartment sink will have a designated compartment (the rinse or sanitize compartment) which will be used for a prep sink. The sink must be wash, rinsed and sanitized before and after use as a prep sink. A sign will be posted stating this.

The 3-compartment sink needs to be sealed to the wall and the gaps between the wall and the duckboard must be eliminated.
