The owner was present during the time of this inspection. Discussed coming into the health department for a category change if he is still wanting to serve food.

    A recheck was conducted on 09/23/15 to determine if the back door had been corrected. During the previous inspection, it was found that there were large gaps on the lower right corner of the door open to the outside. No barrier was present to keep insects and rodents from entering the establishment. During the recheck, it was found that a door sweep has been installed and the door has been fixed to create a tight fitting seal - Corrected. No further rechecks are necessary at this time.

HACCP: Toxic handout and education on storing toxic chemicals away from all food products and utensils

* A sign located next to the two tvs above the cash register was observed to state hamburgers are being served at the establishment on Sundays. Hamburger patties were found in the freezer in back room refrigerator. Spoke with employee who states they grill out on Sundays in the back of the establishment. Due to the category III license currently issued, this establishment is not licensed to serve hamburger or any other potentially hazardous food. This establishment is to cease all sales of potentially hazardous food including hamburgers. If the establishment wishes to sell potentially hazardous food in the future, a visit to the Peoria City/County Health Department is required for further paperwork concerning the potential to change categories.
  • Food contact surfaces of equipment and utensils clean
    The pizza oven next to the microwave is heavily soiled on the inside grill racks.
    All food contact surfaces should be free of debris and build-up, clean, and sanitized at all times

  • Critical: Outer openings protected from insects, rodent proof
    The back door next to the restrooms is missing a piece from the lower right corner. Several gaps in the door can be seen.
    Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight fitting self-closing doors, closed windows, screening, controlled air currents or other effective means.
    Correct by replacing or repairing door. A recheck will be conducted on or after 09/23/15 to determine if the door has been repaired to protect against the entry of insects and other rodents. A recheck fee will be assesed for all necessary rechecks. •

    The floor in the bar area below the 3 compartment sink and behind the soda machine is soiled
    Floors, floor coverings, mats, and duckboard shall be maintained clean.

    No sanitizer detected in "sanitizer solution" at 3 bay sink.  Maintain sanitizer solution concentration at 50-100ppm.  Worker made fresh sanitizer solution.  Corrected.

    Observed single service articles stored in box on floor in storage area behind white door opposite bar.
    Single-service articles shall be stored at least six inches above the floor in closed cartons or containers.
