  • Plumbing installed and maintained
    The Cap to the antisiphonage device to the mop sink faucet is missing. Replace cap.
    Please delime hand sink faucet close to the managers office.  
    The hand sinks to the rest rooms have small amount of build up on basins.
    Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.
  • Floors properly constructed, clean, drained
    Found build up on floor under soda dispensing area. Found excess of water on floor in back prep. Make sure that after potatoes and dishes are washed to dry mop area. Concern is to avoid employees slipping and falling. Floors shall be clean.
  • Walls, ceilings, and attached equipment, properly constructed, clean
    Found paint peeling off the men's restroom door. Please repaint door so that it is easily cleanable.
    Walls and ceilings, including doors, windows, and similar enclosures shall be maintained in good repair.
