HACCP= salad dressing for the salad bar quat sanitizer at the bar = 100ppm
  • Food protection during storage, prepartion, display, service, transportation
    Various food items which are set-out near the salad bar during the lunch period are not being protected. Food shall be protected from the potential for contamination at all times, including while being stored, prepared, displayed, served, or transported. Protect these food items by providing different containers or covers for the currently used containers.
  • Food contact surfaces designed, constructed, maintained, installed, located.
    1) The interior surface of the Frigidaire cooler in the kitchen has a broken shelf on the door which has black tape over it and the bottom drawer is broken. 2) The four door reach-in bar cooler has a large amount of water/liquid on the bottom interior surface. Food-contact surfaces shall be smooth and easily cleanable, free of breaks, open seams, cracks, chips, pits, and similar imperfections, and free of difficult-to-clean internal corners and crevices. Repair/resurface/replace the cooler interior surfaces and properly drain the bar cooler.
  • Food contact surfaces of equipment and utensils clean
    1) The ice guard inside the ice machine is heavily soiled. 2) The bottom interior surface of the Frigidaire cooler in the kitchen is soiled. Kitchenware and food-contact surfaces of equipment shall be washed, rinsed, and sanitized after each use and following any interruption of operations during which time contamination may have occurred. Clean these areas.
  • Critical: Hand washing sinks installed, located, accessible
    1) The hand sink in the kitchen has black plastic baskets and a plate present in it. 2) The hand sink at the bar has beer pitchers in it. Lavatories shall be accessible to employees at all times. The items have been removed from the hand sinks and are now accessible.
  • Restrooms with self-closing doors, fixtures operate properly, facility clean, supplied with hand soap, disposable towels or hand drying devices, tissue
    1) The mens restroom has a urine odor. 2) The soap container at the hand sink is all out. Toilet facilities, including vestibules, if present, shall be kept clean and in good repair and free of objectionable odors. A supply of hand-cleansing soap or detergent shall be available at each lavatory. Clean the mens restroom and provide soap at this hand sink.
  • Floors properly constructed, clean, drained
    The floor in the kitchen is rough in some areas and is soiled. Floors and floor coverings of all food preparation, food storage, and utensil-washing areas, and the floors of all walk-in refrigerating units, dressing rooms, locker rooms, toilet rooms and vestibules shall be constructed of smooth, easily cleanable, durable material. Floors, floor coverings, mats, and duckboard shall be maintained clean. Repair/replace/resurface the floor and claen as needed.
  • Walls, ceilings, and attached equipment, properly constructed, clean
    1) The vent fan cover on the ceiling in the womens restroom is soiled. 2) The vent fan cover on the ceiling in the mens restroom is soiled. 3) A ceiling tile in the mens restroom is stained. 4) Several ceiling tiles in the kitchen are stained. Walls, ceilings, and attached equipment and decorative material shall be maintained clean. Clean the vent fan covers and replace the ceiling tiles which are stained.
  • Lighting provided as required. Fixtures shielded
    1) One of the two lights in the womens restroom is burnt-out. 2) One of the lights on the ceiling in the kitchen is burnt-out. Permanently fixed artificial light sources shall be installed to provide at least 20 foot-candles of light on all food preparation surfaces and at equipment or utensil-washing work areas. Replace the lights.

Complaint #11-0196 which stated that there was no water in the restroom nor would the toilet flush. During this complaint investigation it was found that this complaint is not valid. Both the mens and womens restrooms have running water and the toilets flush.

The owner stated that the water was shut off this past Thursday for about 5 minutes so a pin hole leak could be repaired.

Case closed. No further investigation.
