If the medicine is also one that humans use, you can use GoodRx (it's free!) to get huge discounts on it. Our dog was on desmopressin and we saved 80% on the cost by using GoodRx. While I don't know if it's supposed to be used for dogs, none of the pharmacies we used cared. I remember them asking me for "date of birth" when picking it up and me saying I don't know, it's a dog and they just handed me the drug after I paid the discounted rate.
Another thing you can do is ask if there are any rebates. For example we had a diabetic cat and needed a glucose testing gizmo and I explained that to the pharmacist and he just handed one to me for free. The manufacturer had a rebate that covered the full cost because they wanted to get people to buy their test strips.
I used to get this same heart medication for my chihuahua/schipperke from Drs. Foster & Smith, which no longer exists (they were bought out by Petco & my little girl died in 2017). Anyway, they were actually cheaper than the Costco pharmacy, at the time, & very quick shipping via UPS. I don't know if Petco ships rx for pets, it may be worth looking into.
I forgot to report that the 2nd vet stated that small dogs (ours are shih tzus) commonly get heart murmurs and that it's not indicative of--but may lead to--heart disease. She recommended the app Cardalis to monitor his respiratory rate. >30 or inching towards 30 is cause for concern. Both our 6yr and 13yr olds are measuring between 13-16 breaths per minute.

Our last dog who had heart disease had a heart murmur his entire life and our vet made sure we knew to watch out if he started to cough regularly. Our current dog with heart disease did not have a murmur and he's in the early stages of heart disease.
