NO PEST CONTROL LOG BOOK ON SITE WITH ALL REQUIRED DOCUMENTATION. INSTRUCTED TO CONTACT LICENSED PEST CONTROL TO SERVICE PREMISES AND PROVIDE ALL REQUIRED DOCUMENTATION IN LOG BOOK AT ALL TIMES. SERIOUS 7-38-020 NO CITATION ISSUED.
The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO SEAL OPENINGS AT SIDE WALLS INSIDE SOFT SERVE UNIT CABINET IN MIDDLE SECTION. SEAL OPENINGS AT WALL BASE IN TOILET ROOM NEXT TO WASHBOWL AND REMOVE SODER FROM PIPES AT WALL IN TOILET ROOM.
INSTRUCTED TO PROVIDE HOT RUNNING WATER AT UTILITY SINK AND DECREASE PRESSURE SO AS WATER NOT SPRAYING AND ACCUMULATING ON FLOOR AT REAR PREP AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REMOVE ALUMINUM CAN AT UNUSED FLOOR DRAIN UNDER THREE COMPARTMENT SINK AND REPLACE WITH CAP/FLOOR DRAIN COVER.

DUMPSTER OVERFLOWING AND LIDS NOT TIGHT FITTING. TRASH AND DEBRIS ON GROUND AROUND DUMPSTER. YOGURT SPILLS LEADING FROM ESTABLISHMENT TO OUTSIDE GARBAGE AREA. INSTRUCTED MANAGER TO CLEAN OUTSIDE GARBAGE AREA. SERIOUS VIOLATION 7-38-020 CITATION ISSUED.
All food not stored in the original container shall be stored in properly labeled containers. PROPERLY LABEL ALL BULK CONTAINERS, TOPPINGS, ETC.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. CLEAN INTERIOR AND EXTERIOR OF ALL CABINETS AND SHELVES.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS NEED CLEANING IN REAR (STICKY SUBSTANCE, SPILLS, ETC), ALONG WALLS AND IN ALL CORNERS IN STORAGE AND WASHROOM AREAS.
