The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST REPLACE MISSING AND BROKEN FLOOR TILES IN KITCHEN'S STAFF WASHROOM, AND CLEAN FLOORS THRU-OUT INSIDE MOP CLOSETS AND STORAGE ROOMS.

ONE DEAD MOUSE AND OVER THIRTY MOUSE DROPPINGS WERE FOUND IN THE ROOM UNDER THE STAIRS NEXT TO THE CAFETERIA, ACTIVE RODENT HOLES WERE FOUND ON BOTH SIDES OF THE WALK IN COOLER AND NEXT TO THE STAIRS IN THE EAST END OF THE LUNCHROOM. OVER TWENTY MOUSE DROPPINGS WERE FOUND IN ROOM 115 AND THE ADJACENT WASHROOM AND STORAGE CLOSET. AN ACTIVE RODENT HOLE WAS FOUND IN THE LIBRARY NEXT TO A CLOSET DOOR AND THE DOOR SWEEP AT THE TOP OF THE EAST CAFETERIA STAIRS IS GNAWED THROUGH. REMOVE ALL EVIDENCE OF PEST ACTIVITY; CLEAN AND SEAL UP ALL POSSIBLE AREAS OF RODENT HARBORAGE AND SANITIZE ALL AFFECTED AREAS. REVIEW TREATMENT AND REPORTS WITH YOUR PEST CONTROL COMPANY.

REPAIR THE WALLS AND CEILINGS SO THEY ARE SMOOTH AND SANITARY.
THE PIPE IN THE 2ND FLOOR BOYS SOUTH BATHROOM HAS EXPOSED FIBERGLASS, SEAL UP EXPOSED INSULATION.
A BUCKET OF STAGNANT MOP WATER WAS FOUND IN THE MOP CLOSET ACROSS FROM ROOM 115, BUCKET DRAINED AT THIS TIME. THE SOAP DISPENSER HAS COME OF OF THE KITCHEN BATHROOM WALL, REPLACE THE DISPENSER.
THE METAL STEM THERMOMETER USED IN THE KITCHEN IS 32 DEGREES OFF, AN ICE SLUSH WAS MADE AND I CALIBRATED THE THERMOMETER TO 32 f AT THIS TIME.
