FOUND NO PEST CONTROL LOG. SERIOUS VIOLATION. NO CITATION ISSUED.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 2-COMPARTMENT SINK FAUCET LOOSE. MUST REPAIR AND MAINTAIN SINK.

THE FOLLOWING NOT CLEAN- OBSERVED SLIGHT LIME BUILD UP INSIDE ICE MACHINE AND FOOD BUILD UP INSIDE PREP AREA COOLER DOOR GASKETS. MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED ACCUMULATED FOOD DEBRIS INSIDE FLOOR DRAIN UNDER 3-COMPARTMENT SINK. INSTRUCTED MANAGER TO CLEAN AND MAINTAIN FLOOR DRAINS.
The walls and ceilings shall be in good repair and easily cleaned. MUST CLEAN ALL UNCLEAN WALLS BY REAR EXIT DOOR.
