Violation descriptions and comments

All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. OBSERVED SHREDDED CARROTS BEING THAWED IN FRONT SUSHI PREP EXPOSED HAND WASHING SINK AND DIRTY GLASS WARE ALSO INSIDE SAME SINK AT SAME TIME. INSTRUCTED MANAGEMENT THAT EXPOSED HAND WASHING SINKS ARE FOR HAND WASHING ONLY. FOOD AND DISHES REMOVED.
Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F. OBSERVED CERTIFIED MANAGER WASHING DIRTY DISHES IMPROPERLY AT THREE COMPARTMENT SINK WITH OUT ANY SINKS BEING FILLED AND NO SANITIZING FINAL RINSE. CERTIFIED MANAGER WAS UNABLE TO SAY WHAT IS TO BE USED TO SANITIZE OR ABLE TO DEMONSTRATE THE PROPER WAY TO WASH RINSE AND SANITIZE. I FOUND BLEACH ON PREMISES AND DEMONSTRATED THE PROPER METHOD.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INTERIOR COOLER ON RIGHT SIDE AT FRONT SUSHI PREP WITH FOOD DEBRIS, MUST CLEAN.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MISSING FAN GUARD COVERS IN REAR TALL REACH-IN COOLER. MUST REPLACE.
The walls and ceilings shall be in good repair and easily cleaned. WATER DAMAGED CEILING TILES AT REAR TALL REACH-IN FREEZER AND AT THREE COMPARTMENT SINK. MUST REPLACE.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. THREADED NOZZLE ON REAR HALLWAY MENS WASHROOM MUST REMOVE.

Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. MUST INVERT SINGLE-USE STYROFOAM STORAGE CONTAINERS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED RUSTED WIRE STORAGE RACKS IN THE PREP COOLERS. MUST REPAINT OR REPLACE STORAGE RACKS.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. MUST PROVIDE A SELF-CLOSING DOOR DEVICE FOR THE RESTROOM DOOR IN THE DINING AREA.
All employees shall be required to use effective hair restraints to confine hair. HAIR RESTRAINTS ARE NEEDED FOR ALL FOOD HANDLING EMPLOYEES.
