Above had no proof of pest control service, pest control log book available at time of inspection on 6/13/2011, Citation #H000071344 issued with a court date of 7/21/2011, at 400 w superior in room #112 at 9:00 AM.
Fund food items not properly dated/labeled in walk in cooler, coolers, must date and label food items in walk in cooler, coolers
Walk in cooler shelving that had peeling paint shall be repair/sealed, bsmt. storage shelving that had exposed raw wood shall be repair/sealed.
Non food contact surfaces of cooking equipment, cooler shelving, storage shelving(1st, bsmt floors)need detailed cleaning(crevices).
Floors under heavy equipment, cooking equipment, storage shelving(1st floor, bsmt.)not clean need detailed cleaning(corners).
Damaged, stained ceiling tiles in bar areas, shall be replaced, walls in kitchen prep areas need cleaning.
Light shields in prep area, bathrooms need cleaning.
Exhaust canopy not clean needs detailed cleaning, exhaust vents(ventilation)in walk in coolers, coolers, prep area, and bathrooms need cleaning(detailed).
