Smoking shall be prohibited in all public places except in designated smoking areas. OBSERVED VIOLATION OF CLEAN AIR ORDINANCE. OBSERVED SEVERAL RESIDENTS SMOKING CIGARETTES IN THE BASEMENT ROOM LABELED AS THE 'LOWER LEVEL SMOKING ROOM.' MANAGEMENT WAS INSTRUCTED THAT SMOKING IS PROHIBITED INSIDE OF THE BUILDING AND OUTSIDE LESS THAN 15 FEET AWAY FROM ANY DOORS, WINDOWS, OR VENTILATION INTAKES. MANAGEMENT INSTRUCTED THE RESIDENTS TO EXTINGUISH THEIR CIGARETTES, LEAVE THE ROOM AND SMOKE OUTSIDE MORE THAN 15 FEET AWAY FROM THE BUILDING DOORS, WINDOWS OR VENTILATION INTAKES. MANAGEMENT LOCKED THE DOOR TO THIS ROOM AND REMOVED THE SMOKING ROOM SIGN. CITATION ISSUED UNDER 7-32-015.
The walls and ceilings shall be in good repair and easily cleaned. DETAIL CLEAN THE CEILING OF THE PREP AREA ESPECIALLY ABOVE THE STEAM TABLE.
OBSERVED A DRAIN LINE WATER LEAK AT THE SPRAY SINK NEXT TO THE DISHMACHINE. MUST REPAIR THE LEAK.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR COLD WATER KNOB LEAKING AT 3 COMP. SINK, AND SLOW DRAINING ON EXPOSE HAND WASH SINK BY 3 COMP. SINK IN KITCHEN.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. INSTRUCTED TO REPAIR RUST ON THE FOLLOWINGS: WALK IN COOLER FLOOR , TOP OF GREASE TAP,AND STORAGE SHELVES BY DISH MACHINE. REMOVE FOIL LINING ON STOVE AND ON GREASE TRAP.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. REPAIR LIGHT AT DISH MACHINE AREA TO BE BRIGHT, AND REPLACE OUT LIGHT AT SERVING LINE IN KITCHEN.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. INSTRUCTED TO STORE CLEAN UTENSIL ON COUNTER BY DISH MACHINE NOT TO HAVE CONTACT WITH WALLS. PROVIDE PLASTIC OR STAINLESS PROTECTION ON THE WALL.
